You have ptosis of both upper lids, worse on the right compared to the left. This can be fixed by tightening the muscles of the lids to help improve the height. You may also benefit from skin removal depending on your goals with surgery. Best to see an eyelid expert.
If you're concerned regarding your upper eyelids having some slight asymmetry - right with the extra fold - then, you may wish to consider consultation for a blepharoplasty. To create even heights in the folds, both sides may need to be addressed; balancing one side to match an unoperated side is challenging.
Hello @nacho cheese, thank you for your question. Body parts do not all have the same size or shape. There are generally certain asymmetries. For example, one eyebrow is different from the other and the same happens with our eyes. Having said that, there are non invasive treatments that work together to improve those asymmetries. As we age, our eyelids naturally sag and droop. A combination approach would be best to see visible improvement. In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. I typically combine this with Ultherapy and thread lifting to lift the brow; RF and skin tightening laser treatments to stimulate collagen production and rejuvenate the skin; and laser or superficial RF devices for improvements in texture, color, and any crepey skin around the eyes as needed. Any type of botulinum toxin would help treat the wrinkles. You might need a full evaluation by a real plastic surgeon before proceeding as you have some element of brow ptosis and excess upper eyelid/lower eyelid skin.
