Female, age 70, excellent health. Needs facelift but want to know if rhinoplasty can fix this nose (first, after, or during). Scar on top of nose (tip) and it aging badly

A lateral crural strut graft can support your nose where this indentation is present. Cartilage from your septum can be used.

Difference between ear and rib cartilage for rhinoplasty?

Rib is more abundant but more challenging to harvest and painful for the patient. Ear cartilage is less abundant and can lead to ear deformities. Septal cartilage is the best. Fresh frozen cadaveric rib cartilage is seeing significant use.

The reaction you described with lidocaine with epinephrine is an expected effect as epinephrine has a known effect to increase heart rate. If you are under general anesthesia, then this will not be a problem as you will be monitored throughout the case. We would not expect any shaking of the...
