I resisted getting surgery to restore my breathing for years. Procrastination only prolongs your misery and never solves any problems, only taking action does. Constant snoring, mouth breathing and the eventual collapse of my nasal valves upon respiration was beginning to be too much of a burden as I aged. All this in combination with a shrinking nose was about all the motivation I needed to finally take some action. After my 60th birthday, I decided it was time to do something about it because I'm not getting any younger. The clock is simply ticking away, and I want to live healthier for my remaining years on this earth. Rhinoplasty is Dr Dibelius's sole specialty, and it shows in large part in the results of his patients. I developed a keen eye for nasal aesthetics over many years and I could quickly dismiss a surgeon after just viewing a sampling of their work and I was able dismiss countless surgeons just by viewing their websites; needing to see only 10 or so of their patient's results. I always knew when someone had surgery on their nose because there were telltale signs left behind by less qualified surgeons and even when no one else could tell, I always could. My result today by Dr Dibelius is beyond my expectations and who could ask for more than that? The result is essentially the same nose I was born with but more refined looking and in complete harmony with my face. I breathe like a champion racehorse which motivated me to take up running again. Dr Dibelius understands the difference between a surgical appearance and a natural appearance. Notwithstanding my original God given nose, I never looked more natural or better than I do now and the result in function and aesthetics is excellent (to say the least). A special thanks to Amy and Sophia for all their genuine caring work, follow-ups and administrative work behind the scenes. These two women were like family to me and treated me in loving way during my whole journey. In conclusion, I'd advise you to do your own due diligence so don't take my word for this but just do research, investigate and consult with at least 3 surgeons; but do yourself a favor and consult with this highly trained surgeon as at least one of your options. I suspect it will be one of your best decisions you ever made if you want to minimize your risk and obtain a beautiful result you can live with for the rest of your life. IMHO, it is an understatement to say Dr Dibelius is a rhinoplasty genius and I am blessed and grateful to have been his patient.
This has been an amazing experience for me. Dr. Dibelius is a skilled and talented surgeon who is also kind and thoughtful. He listened to my concerns and took his time answering every one of my many questions. Amy, his patient coordinator, has also been incredibly helpful throughout this process, answering questions and returning phone calls in a timely fashion. I also want to mention Sophia, who was also extremely helpful. As I write this, I am just about 5 months post revision rhinoplasty and I am thrilled with my results. Dr. Dibelius and his staff are the best! I had an amazing experience from start to finish. Sofia was able to book me for a consultation very quickly and I got in to see Dr. D and Amy, his patient coordinator, who both met with me for over two hours to go over the procedure (a septoplasty to correct my deviated septum with some minor aesthetic modifications to my nose as well), take pictures of my nose, upload them, and get my input on aesthetic changes I wanted to make to my nose. From there, Amy and Sofia helped with insurance and financing, booked the surgery, and stayed in close contact with me through the surgery date and post-op. I had a moment of panic a couple of days before the surgery because I have get anxious when I’m not able to breathe through my nose and worried that the congestion after surgery would be unbearable for me. When I expressed my concerns, Amy called me that same day to reassure me, and made me feel much more comfortable. The day of the surgery, I met with Dr. D in pre-op and he also let me know he’d do what he could to minimize the congestion by placing the thinnest splints possible in my nose. Immediately post-op, I could feel a huge difference in my ability to breathe. The recovery overall was very easy. I had no bruising and little swelling, and minor congestion. Dr. Dibelius clearly uses a very light touch. He took my cast off after a week, and I couldn’t be happier with the aesthetic results either even though my nose is still swollen—my nose looks incredibly natural—just like my nose but better. My case was quite complex; in fact I was turned away by my local plastic surgeon because this would be a complicated revision septoplasty after 2 failed surgeries from an accident I had years ago. I had a deviated septum on the right, a collapsed left side, breathing issues and a crooked nose. Dr. Dibelius didn’t blink an eye at my case, and assured me that he could make me whole again. Dr. Dibelius is an amazing, knowledgeable and very skilled surgeon. I am 3 months post revision rhinoplasty and I could not be happier with the results. I was so afraid to have another rhinoplasty 20 years after the first one but was so unhappy with the "pinched" and elongated look of my nose. Dr. D spent a full hour with me in our first meeting going over what my desired outcome was and what his professional opinion and approach would be. It was important for me to find a plastic surgeon that did only rhinoplasties and who was experienced in revision rhinoplasties, as I knew it would not be a straight forward typical surgery. Dr. Dibelious is exactly this person. He called me the day before my surgery to check in and go over everything again and always asked if I had any questions or concerns. The surgery went very well, a cartilage graft from my rib was utilized and a deviated septum was also repaired. Dr. Dibelius performed a complex rib-graft rhinoplasty to improve some cleft-palate related issues that were affecting the shape of the nostrils and nasal tip in particular. I had two procedures as a teenager that failed to make as big of an improvement as I had hoped and also made the prospect of an additional procedure even more delicate. But Dr. Dibelius exceeded my expectations and I doubt there are many surgeons in the country who could have achieved such a strong result. Not only were the results great, but he was also exceptionally clear and communicative throughout the entire process, and worked diligently to quickly schedule my procedure during a narrow window of time when it worked best. Approximately six months after the procedure, I'm still blown away by the results, and Dr. Dibelius still schedules semi-regular checkups to monitor the results at no additional charge. If your situation is particularly complex or unique, I cannot recommend Dr. Dibelius enough. He is an expert and a professional.

Unlike many other surgeons who dismissed my concerns and questions about surgical techniques and anxiety related to revision rhinoplasty, Dr. Dibelius displayed exceptional care and confidence right from our initial consultation and continued to do so throughout subsequent follow-ups. He dedicated ample time to thoroughly address all my inquiries, providing unwavering assurance. My journey with Dr. Dibelius began when I was emotionally struggling after a previous septorhinoplasty performed by another doctor. This procedure was intended to address severe breathing difficulties resulting from sports injuries and assault to the nose, but it left me feeling like my ethnic identity was erased and my nose was botched. The previous surgeon disregarded my worries and denied the evident outcome, offering a revision with ear cartilage as a remedy. Hi, One of the key elements of performing a good revision surgery is that you and your surgeon see eye-to-eye on the problems and the proposed solutions. If the surgeon doesn't see an issue, then the chances are low that they will be able to correct it. You did the right thing by asking your original surgeon first, but sometimes it is necessary to get some different opinions, just like in all aspects of medicine and life. Try to identify some surgeons that specialize in revision rhinoplasty, since they will be able to give you the best advice. Just be aware that many will ask you to wait a few more months to allow for additional healing prior to consultation. Good luck to you!
Hi, Revision rhinoplasty is nearly always an option if done right and in the right patient. What I mean by that is that many things can be corrected, but may need rib cartilage for example to rebuild the nose. If the patient is motivated and willing to do this type of procedure then it can absolutely work and have great results. It is very rare for the nose itself to be so unhealthy that revision is too risky, and in those patients for which there is moderate concern for the health of the nasal soft tissues there are other things that can be done to help reduce the risk (fat injections, hyperbaric O2, etc.) With regard to the cartilage hanging inside the nose, this would require a detailed physical examination to determine what is going on there. I recommend speaking with the surgeon who performed the surgery first to see what the initial assessment is and if anything needs to be done. I think a revision surgery would potentially be an option for you if you wanted to seek it out. Make sure to speak with a specialist who does a lot of revision rhinoplasty as it is more complicated than primary rhinoplasty in most cases. And of course, make sure their credentials include board certification in plastic surgery or facial plastic surgery. I hope this helps!
Hi, It would be worthwhile to know how long it has been since surgery. In the initial healing period scar tissue forms and there is a significant interplay between scar formation and transient swelling. We deal with the swelling and early scar tissue primarily with kenalog injections, but also with things such as taping, nasal massage/exercise, diet modification, behavior modification, medical management of infection. If surgery was a while ago and the initial healing/scar phase is completed, then it becomes more likely that a revision surgery may be needed to correct this. It may be still worthwhile to try a kenalog injection if you haven't already though. Make sure you speak to a qualified, board-certified plastic or facial plastic surgeon to help you sort this out. Hi, I generally think that if you are healthy and the skin and soft tissues of your nose are healthy, then it is reasonable to undertake a revision rhinoplasty, even if there was more than one surgery before. You do have to allow for sufficient time for the nose to heal; if your last surgery was in 2019 then that should be plenty of time. However, I would say that these surgeries usually require a different approach, and frequently that means using some other source of cartilage like ear cartilage or rib cartilage in order to get the desired result. This would be the biggest difference between a primary surgery and a revision surgery. Ironically, typically we need to add these cartilages in the form of well-placed grafts to actually make the nose appear smaller and thinner. You would need to consult with a board-certified facial plastic surgeon to review more specifics of whether you are a good candidate for surgery, but just based on your description and photo, revision is definitely a potential option. I would also encourage you to look for surgeons who focus on and have expertise in revision rhinoplasty, as they would be able to provide much more detailed and specific information to your case. You will almost certainly find different answers if you speak to multiple surgeons and so it is definitely worthwhile to speak to as many as you can. There is no need to "settle" until you have gotten all of your questions answered!
In cases where there is a "pointiness" on one side of the tip associated with a different nostril shape, I would guess that the underlying issue may be an asymmetry of the tip cartilages. This theory is supported by the fact that if you push down on the pointy side it helps fix the nostril issue. One slight caveat is that at 10 months out there is still some swelling reduction that needs to occur, although it is a decent time point to make an assessment. However, I want to make the point that in most cases I would avoid doing a minor procedure to trim down the pointy part of the tip. The tip cartilage that runs on that side is a continuous piece that goes from out laterally near the cheek, then making a sharp curve at the pointy part of tip as it goes down into the middle columella area. Because of this, trimming the middle dome portion might actually make things worse, turning a small problem into a bigger problem. Although it may reduce the "pointiness", it may damage the integrity of that cartilage and make the nostrils more uneven, as the cartilage becomes weakened by the procedure. There is one situation that I can think of where it might be ok to attempt a minor procedure, and that is if the bump is actually a graft that has not healed in quite the right spot. Only your surgeon is going to know if that is the case, so I would continue to try and reach out to them. In summary, I would encourage you to get in touch with your surgeon or to seek out a second opinion consultation to help you make a decision on this. But I would strongly recommend that you avoid doing any potentially destructive procedures until you have a clear plan for your nose.
