Hi, I really want a breast augmentation. (implant + lift) I have done several consultations and they are all telling me different things. One surgeon that I really like wants to do an anchor incision for best results, but then another surgeon says that a donut incision would be best. and a third said that I don’t need a lift. Can you you lift the breasts up enough with a donut incision? I’d love any advice on how to go forward! Thank you!

Answer: Breast lift Hello glambrina1.You definitely do need a lift. We would need to go from a periareolar lift (donut lift) to an inverted T scar. That will depend on the size of implant that we would use. The smaller the implant, the more skin we need to take out to have the breast well adjusted, and the more scar we need (inverted T). The bigger the implant, the less skin we need to take out and the less scar (periareolar lift). If we use a medium size implant I would start with a periareolar lift, but if I see that there is still loose skin I then add the vertical scar and the horizontal scar if needed.We would have to determine what type of result you want and what scars you are willing to accept. Regardless, it is clear without actually examining you that your nipples are below your inframammary fold and point downward (classic Grade III ptosis). Doctor No Lift is interested in your money more than your results. Doctor Donut is hoping that you will look OK, and that your periareolar skin excision doesn't do nasty things like pleat, expand the areola, flatten the breast, or develop an extremely wide scar.
