Hi. posted a question 2 months ago. Tried to do update but can't figure it out. I have perioral smoker lines and small 7 month atrophic dog bite scar. Did Botox and Restylane Silk. I asked not to inject scar because wasn't sure it was 100% healed but they did it and the surrounding wrinkles. 2 months later I see no difference in the appearance of my smoker lines and I think my dog bite actually looks worse, its puffed on the edges and longer. Maybe I need a more aggressive approach. I don't like the filler.

While we use fillers periodically for smokers lines, we always let patients know that a better and longer lasting option is laser resurfacing. Laser resurfacing can remove the lines whereas fillers are just camouflaging. Before and after photos below that are similar to your situation show the outcomes you are looking for.

Answer: Treatment of Smoker's Lines Around Mouth The smoker's lines around the mouth are difficult to treat and always respond well to a combination of treatments. We use Botox, Fillers, Microneedling, Radiofrequency, Ultrasound, Chemical and Laser peels to treat the area. If you are not happy with the fillers then I'd recommend you consider a laser treatment which in my opinion provides the best improvement. Make sure you see an experienced physician for an in-person consultation and evaluation. Best wishes.

This is a useful modality to help with some of those stubborn fine lines and wrinkles that are difficult to fill. Also take a look at any habits you have that might be making the lines more pronounced, such as drinking from a straw or small mouth water bottles, which are both helpful to decrease. Good sun protection and retin-a at night would also be critical to keep the skin healthy and less sun damaged. Botox in the lip area tends to not last too long, so often patients need to have a few units injected every 8 weeks or so for more long term improvement. See an expert and best of luck!

Answer: Microneedling for scars Hello, Great question and thank you for sharing your picture. A combination of treatments would be best for your concerns including microneedling and filler. I would recommend a series of microneedling (3-4) treatments with PRP and possibly maintenance treatments. Also, the use of low dose retinol at home every other night to help with skin texture and appearance. For a more aggressive treatment, recommend C02 treatment with a downtime of 7-1o days. If there is still a bit of atrophy, a superficial filler like Volbella can be used to help further soften the more persistent wrinkles/scars. Even more aggressive would be a laser treatment, like Halo, to help improve perioral lines and scarring without use of injectables. Hope this helps!
