28, I do not plan to have children soon. Breast lift May 2017 (no implants) *I'm incredibly happy with the breast shape. Unhappy with nipple placement. May 2017- Revision to lower both nipples, by removing skin at the bottom of the inframammary fold. *My nipples, especially my left are still so high that they show even very modest sports bras that are not low cut. *The skin above my nipples is quite short in distance and very tight.
Answer: High Riding Nipples Hello, It is unfortunate that you didn't get satisfaction from your revision surgery, and only your surgeon knows exactly what he did. What should have been done would include reducing the internal capsule (capsulorrhaphy) so that the implants are positioned higher on your chest, without the support of your skin to keep them there. In addition, an external reduction of skin from the inframammary fold would tighten up and inframammary skin. By combining these two procedures, your implants should have less lower pole fullness and hang, and more upper pole fulness. Best of luck!

Answer: Too much bottom breast Lowering the areola from above will not work because the resulting scar will then pop out of the bras... The bottom of your breast is too low and that's where the brawl wants to settle. The key will be changing the bottom of your breast to a higher position so the bra will now sit at that level and cover the areola. That means a real successful revision and reduction of that bottom part of the breast. That should be discussed with your surgeon!!!!

High-riding nipples are often difficult to correct. However, given your submitted photo, I would suggest excising a wedge of skin and soft tissue at the inframammary fold and create an inverted 'T' incision. It appears that the nipple to inframammary fold distance is quite long and should be shortened. This would hopefully ameliorate your concerns to some degree. Hope that this helps! Best wishes!

Answer: Nipple areolar position It seems like you already had inappropriate surgery to try to correct this problem. The situation itself is well-known to be difficult to correct. There may be other approaches to try to fix this, including tissue expanders, etc., but this can get very complicated and increase the risk of other problems.

An examination would better help to determine if and what may be done to improve your situation. One possibility, again depending on the residual laxity of your tissues would be to reduce your breast size by removal of tissue lower down and removing an ellipse of skin and tissue near the bottom, allowing to rotate your nipple down to a more satisfactory position. This will also avoid a scar higher up.You should seek one or more second opinions from reputable board certified plastic surgeons and then make your decision accordingly.Steven Turkeltaub, MD
