Nasolabial fold excision before or after face lift?

Excision of the nasolabial fold can occasionally cause an unsightly scar and that is the reason it is mostly avoided these days. Instead, volume replacement and proper lifting can be quite effective in addressing the age related changes of the fold. It should be noted that rejuvenation of the melolabial fold is classically considered one of the more challenging areas in facial rejuvenation.

Nasolabial fold excisions are usually done in severely indented folds, not likely to respond to a facelift. They do not affect other facial aging issues.The facelift indications are usually droopy cheeks and saggy necks. If these 2 issues are present in your case a face lift would probably be indicated to correct these issues if important for your final results. I would wait a minimum of 3 months before undertaking a lift.

Excision of nasolabial folds can be a useful addition to a facelift.  I perform it only occasionally and perform it at a secondary procedure.  Most of the patients I perform this on are older men.

Nasolabial fold excision can be necessary in some instances, but usually a good subperiosteal mid face lift can eliminate the necessity for this.  The fat pads of the cheek, when they fall, often sag over the fixed ligaments of the fold.  By repositioning these pads, the sagging of the face over the folds can restore a natural more youthful look.

Without seeing you in person, these are just general recommendations, but I would recommend doing a facelift and seeing if you even need excision of the nasolabial folds. I would not do them at the same time. Excision of nasolabial folds is not very common given the scar location, and can be improved with other techniques.

I would have the facelift done first. i'm not crazy about having them both done at the same time because the blood supply might be a little compromised from the excision. Also the facelift might improve the nnasolabial folds. Your surgeon can probably be a little more accurate doing later. Excision of the folds is a much simpler office procedure.

I would recommend you pursue the facelift.  If properly done this help improve your cheek ptosis and efface your nasolabial folds.  You may not require the NL fold excision after all.  

Thank you for your question.Facelift will help improve upon the nasolabial folds and you may not even need an excision done in that area. Nasolabial excisions are not a very common procedure anymore, because of other surgical options. A facelift with fat grafting to the nasolabials will help tighten the skin and improve the folds, this will help eliminate having to do the excision and there will not be a scar in the folds.

Nasolabial fold excision was a procedure used more frequently before the introduction of dermal fillers.  If you are having a facelift performed, I would first wait to see the amount of correction provided by your procedure.  If the folds are still an aesthetic issue, you can then consider excision or fillers based on your preference.
