I’m 5’, 115 pound. I’m a reconst. patient (bilat. mastectomy in 2016). I have the Gummy bear shaped implants (345 cc on each side), which offer a nice natural look. However, I have rippling and no cleavage (which I hate!). I’m going in for revision work, and the new surgeon I met with is suggesting the round smooth overfilled implants by mentor to give me more cleavage and less rippling. My only concern is that I won’t look natural. Should I be concerned? Is this the right direction to go?
Answer: Most American Surgeons Like Round and Smooth Hello,Your initial reconstructive surgeon is familiar with what the rest of the world uses most: shaped and textured implants. Your new surgeon is more in line with 90% of American surgeons: round and smooth, it's all they really know. You are incorrect that your shaped implant is to blame for your lack of cleavage; that's a common misconception proliferated by surgeons, along with the opposite assertion that round and shaped implants give the same results; the truth is that it's not that simple or black and white. Simply overfilling an implant like the new Mentor model does not necessarily make things better, and in fact this new 'x' model from Mentor ripples just sitting on the table top; I must say it was a disappointment when the rep brought it by for me to see. If you were to change your implants to round and overfilled, I would steer you towards a more cohesive Inspira implant, however I think a more cohesive shaped gummy bear implant like the Natrelle 410 in a larger size will ripple less and give you more cleavage. A round implant will make you look different and I am not sure the rippling will be any different. The space between your breasts can be minimized by a wider implant. For more upper pole fullness/cleavage will be obtained with a rounder implant.

Answer: Breast Reconstruction after Mastectomy Unfortunately that is one of the problems with the Mentor implants used in breast reconstruction: rippling. Even with the CPG/Shaped implants of mentor they do ripple. The smooth round Xtra implants of Mentor would still ripple in breast reconstruction patients who are very thin and don't have much subcutaneous coverage.To address this there are several options that I have found may have to work together. The use of ADM (acellular dermal matrix) with fat grafting and using Allergan 410 or Cohesive Rounds would be the best way to address this issue. Mentor implants don't have the cohesiveness that the Allergan 410 implants do.I would recommend you consult with a board certified plastic surgeon who has a lot of experience in breast reconstruction after mastectomy.Hope that helps.

I typically don't recommend these unless you have very small breasts to begin with or have had a mastectomy. I see no advantage to a round form stable implant. Generally if you want a round implant its best to not go with a form stable implant to allow more natural look and feel. Form stable implants are much more firmer. I carry all sizes and types of implants in my office to allow my patients to get a better sense for their implant choice.
