Gunshot injuries can cause both fat necrosis and also tethering of skin to muscle, Scar contraction etc. A physical examination can assess it better. There are various option from scar tissue excision, subscision, fat grafting to correct contour deformities. Consult a board certified plastic surgeon.

In my opinion, the simplest way to improve this area would be to use a filler. You can determine if it will be successful by pulling on the edges of the scar and seeing if the scar lifts. If the scar doesn't lift, you may need a simple procedure called subcision to break up the tethers that are holding the scar. Then, Try a short-term filler first to determine if you are happy with the results. You can then continue the short-term filler or try something like Belafil which will give you permanent results.

Thank you for your question and photo. It is a little difficult to tell how large the irregularity is on your thigh. If it is fairly large and hard, usually a fat transfer is the best option. This will help release the scar and soften the area. If the area is very small, using a product like Renuva would be a good choice. This is an allograft that is transplanted from one person to another. An in-person evaluation would allow you to decide which option is the best for you.
contour irregularities or indentations secondary to trauma are typically caused by far necrosis(fat died),scar tissue and or both. Scar tissue usually shrinks causing the skin to be pulled down chores the area of injury. Generally situations like you described have both loss of fat cells and scratches you pulling the skin down. When that’s the case correcting this by simply adding volume is sometimes effective and other times not so much. The more contracted scar tissue is involved the less effective adding volume such as fat grafting or fillers will work. If there’s a lot of scar issue then releasing it surgically during the fat transfer procedure can improve the results. Thank you for the question, a scar is extremely difficult to manage and one of the best option is to undergo fat injections. Fat can be taken from your abdomen or thigh and it can injected under the scar it usually gives excellent results, however two or three stages may be required as often the fat gets absorbed during the first stage almost 50 to 60% fat gets absorbed, however at the end of two to three sessions you results will be very satisfactory
