QUESTIONS ANSWERED

Nose indentation from nose piercing? (photo)

Asked By:AnonymousANSWERS (1)
Several modalities may be helpful for your nose. 1.Hyaluronic Acid or Other Dermal Fillers Purpose: Non-surgical correction of nasal depressions or irregularities. Mechanism: Injectable fillers can restore volume and contour, lifting depressed areas by filling the space beneath the skin. This can help mask visible deformities due to cartilage collapse. 2.Subcision Purpose: Release tethered scar tissue contributing to nasal depression. Mechanism: A needle or specialized tool is used to break fibrous bands beneath the skin that pull it down, allowing the tissue to lift and creating a smoother contour. 3. Enerjet (Jet Volumetric Remodeling) Purpose: Stimulate collagen production and tissue remodeling to improve skin texture and volume loss. Mechanism: Uses high-pressure jet injections to deliver hyaluronic acid or bioactive compounds without needles. This stimulates deep dermal remodeling and filling of the depression. Combination Approach Filler alone or in combination with Enerjet and Subcision for tissue remodeling. Complex Cases with Cartilage Collapse: May require a surgical consultation for grafting or more invasive reconstruction if non-surgical approaches are insufficient. Good luck. My hair after 14 months in finasteride and 5 months on oral min is stilll thinning

Asked By:AnonymousANSWERS (1)
There are negative Effects of Thyroid Medication and Hypothyroidism on Hair Growth and I believe this could be one of many possible etiologies affecting you. You could be suffering Increased Hair Shedding (Temporary Telogen Effluvium): Here we see that adjusting thyroid hormone levels can shock hair follicles, causing temporary shedding before regrowth stabilizes. Furthermore, You could be sensitive to the Medication. Some individuals react poorly to levothyroxine or other thyroid medications, experiencing persistent hair loss despite treatment. Hypothyroidism and its treatment can negatively impact hair growth in different ways. Here’s how it can affect your hair health: Negative Effects of Hypothyroidism on Hair Growth: It can preciptate Hair Thinning and Shedding: Low thyroid hormone levels slow down hair follicle activity, leading to thinning hair or excessive shedding (telogen effluvium). Based on your history you could have Slowed Hair Growth. Hair grows in cycles, and an underactive thyroid can prolong the resting phase (telogen), delaying regrowth. You may have Nutrient Deficiencies: Thyroid conditions can cause deficiencies in iron, biotin, zinc, and vitamin D, which are crucial for hair growth. An evaluation by a hair expert is suggested including biopsies and bloodwork. Genetic testing is now available to direct your treatment plan. Good luck. Newa+ at home device and fat loss

Asked By:AnonymousANSWERS (1)
The NEWA + device is primarily a radiofrequency (RF) skin-tightening device designed for collagen production and skin rejuvenation. It delivers 10 watts of energy in the form of monopolar RF technology, which targets the dermis (middle layer of skin) to stimulate collagen production and improve skin elasticity. The NEWA + (10W) does not cause fat loss, as its energy level is insufficient to heat fat cells to the point of destruction. Instead, it is effective for skin tightening and wrinkle reduction. What's the best treatment for papular/hypertrophic acne scar on nose? (Photo)

Asked By:AnonymousANSWERS (1)
Bleomycin, a chemotherapeutic agent, has been effectively used off-label in dermatology for treating various skin lesions, including papular scars on the nose. The advantages of using bleomycin for such scars include: Effectiveness in Scar Reduction: Bleomycin works by inducing apoptosis (cell death) and fibrosis modulation, which can help flatten and smoothen raised papular scars. Non-Invasive Administration: Bleomycin can be delivered via techniques like intralesional injection or microneedling, which are less invasive compared to surgical methods. Minimal Systemic Side Effects: When used in localized low doses, bleomycin carries a low risk of systemic side effects, making it relatively safe for small, targeted areas like the nose. Targeted Action: The medication is highly effective in targeting abnormal fibroblast activity in scar tissue, thereby improving the texture and appearance of the skin. Alternative for Resistant Scars: Bleomycin is often effective for scars that do not respond well to other treatments such as corticosteroids or laser therapy. Cost-Effective: Compared to some laser treatments or surgery, bleomycin is a more affordable option for managing papular scars. No Downtime: The procedures associated with bleomycin typically have minimal recovery time, allowing patients to resume daily activities quickly.. Find a scar expert. What procedures should I consider to flatten my forehead scar? (Photo)

Asked By:AnonymousANSWERS (1)
Enerjet is a non-surgical treatment option for improving the appearance of scars, including forehead scars. It combines a proprietary air-pressure technology with the delivery of bioactive compounds like hyaluronic acid or other regenerative substances to stimulate skin repair and remodeling. Jet Volumetric Remodeling (JVR): Enerjet uses high-pressure, needle-free technology to deliver tiny jets of therapeutic agents directly into the skin. These jets penetrate the dermal layers, breaking up scar tissue and stimulating natural healing processes without cutting the skin. Scar Subcision: In the case of scars, such as those on the forehead, Enerjet can function as a form of non-surgical subcision. The force of the jet disrupts the fibrous bands tethering the scar to underlying tissues. This allows for smoother skin texture and improved elasticity. Skin Remodeling: The introduction of bioactive substances (e.g., hyaluronic acid) promotes collagen production, improving the overall appearance of the scar by restoring skin volume and smoothness. Minimally Invasive: Enerjet is needle-free and does not require surgical intervention. Short Recovery Time: There is minimal downtime, with most patients experiencing mild redness or swelling that subsides within a day or two. Effective Results: Patients typically notice a gradual improvement in skin texture and scar appearance over several weeks to months. Multiple sessions may be required. Enerjet is particularly suitable for individuals seeking non-invasive solutions for scars or skin rejuvenation without the risks associated with surgery. Consult an expert dermatologist. CO2 RE laser or blepharoplasty - Which procedure would address under eye wrinkles and volume loss long term? (photos)

Asked By:AnonymousANSWERS (1)
Treating contour loss and periorbital wrinkles involves a combination of dermal fillers and CO2 laser resurfacing, each addressing specific aspects of aging around the eyes. Dermal fillers are used to restore lost volume and smooth out the under-eye hollows, tear troughs, or other areas where the skin has thinned or sagged. Hyaluronic acid fillers (e.g., Restylane, Juvéderm) are commonly used for the delicate skin around the eyes due to their smooth texture and reversibility. The filler is injected into the areas with contour loss to create a more youthful and rested appearance. Careful placement is crucial to avoid puffiness or overcorrection. Immediate improvement is noted, with effects lasting 6 to 18 months depending on the product used. This is Non-surgical, with minimal downtime, and instant results. CO2 fractional laser is highly effective for treating fine lines, wrinkles, and skin texture, especially in perorbital area. The laser delivers precise, controlled heat to the skin, creating microscopic columns of damaged tissue that stimulate collagen production and encourage new skin growth. This smooths out wrinkles and improves skin elasticity. This is typically done under local anesthesia. Multiple passes may be required for deeper wrinkles. Redness, swelling, and peeling for several days to weeks are to be expected.. Full results are visible after 2-3 months as new collagen forms. This gives a Long-lasting improvement in wrinkles and skin texture, as well as an overall tightening effect. The results are Synergistic.....Fillers address the volume loss, while the CO2 laser improves skin texture, tone, and fine lines. Together, they provide a comprehensive rejuvenation of the periorbital area. Find an expert. Jeff Rapaport MD FAAD Realself Hall of Fame Castle Connolly Top Doctor

I have a deep acne scars atrophy (roiling, box scars) had 5 time subcision and I wonder if I need more subcision? (Photo)

Asked By:AnonymousANSWERS (1)
Replacing lost facial fat with fillers can offer several benefits, particularly in restoring a youthful appearance and enhancing facial contours. Here are some key advantages: Smoother Skin: The loss of facial fat often accentuates wrinkles, acne scars and fine lines. Fillers can smooth out these lines by plumping up the skin from beneath, leading to a more rejuvenated look with acne scars. Enhanced Facial Contours: Fillers can be strategically placed to enhance and define facial features like the cheekbones, jawline, and chin. This can create a more balanced and aesthetically pleasing facial structure diminishing the effect of acne scars. Non-Surgical Option: Unlike surgical procedures like fat grafting or facelifts, fillers are minimally invasive, requiring little to no downtime. This makes them an attractive option for those looking for quick results without the risks associated with surgery. Natural-Looking Results: When administered by a skilled practitioner, fillers can produce subtle, natural-looking results that enhance rather than alter a person’s appearance. Customizable Treatments: Fillers come in various types and can be customized to meet individual needs, whether it's restoring volume, filling in deep lines, or refining facial contours. Overall, replacing lost facial fat with fillers can significantly improve facial aesthetics, providing refreshed appearance to a face with acne scars. Is this scar tissue from LPP or other type of hair loss or is it normal? (Photos)

Asked By:AnonymousANSWERS (1)
Your photos show Early Androgenetic Alopecia. No signs of LPP. **Decreased Hair Density**: There is a reduction in the number of terminal (thick and pigmented) hairs. **Increased Vellus Hair**: A higher proportion of vellus (fine and short) hairs compared to terminal hairs, indicating miniaturization, a key feature of AGA. **Altered Anagen/Telogen Ratio**: An increase in the percentage of hairs in the telogen (resting) phase and a decrease in the anagen (growth) phase. This change reflects the shortened growth phase typical of AGA. **Decreased Hair Diameter**: The diameter of individual hairs decreases, further contributing to the appearance of thinning hair. Additional Treatment Suggestions for you: **Low-Level Laser Therapy (LLLT)**: This non-invasive treatment uses laser devices to stimulate hair follicles and promote hair growth. It's considered safe and can be used in conjunction with other treatments. **Platelet-Rich Plasma (PRP) Therapy**: PRP involves injecting your own platelets (and exosomes) into the scalp to stimulate hair growth and improve hair density. It's considered an excellent treatment and may be used in conjunction with other therapies to provide better results. ( Combination Therapy) Find a hair specialist for a comprehensive examination including extensive history, blood work and possibly Biopsy. Is PRF effective for making hair grow?

Asked By:AnonymousANSWERS (1)
Platelet-Rich Plasma (PRP) therapy is increasingly being used as a treatment for hair loss, including hair loss at the crown. Several studies have shown that PRP can be effective in treating androgenetic alopecia, the most common type of hair loss, which includes hair thinning at the crown. - Patients often experience increased hair density, improved hair thickness, and a reduction in hair loss. - PRP is thought to promote hair growth by increasing blood supply to the hair follicles, enhancing the thickness of the hair shaft, and triggering natural growth factors and stem cells that are crucial for hair regeneration. - PRP treatment for hair loss typically involves multiple sessions, often three to six treatments spaced about a month apart, followed by maintenance treatments every few months. - Results can vary based on individual factors such as the extent of hair loss, age, and overall health. Some patients may see noticeable improvements in hair density and thickness within a few months of starting treatment. - While PRP is generally safe with minimal side effects, its effectiveness can vary. It is not a guaranteed solution for everyone. Find a PRP expert. Jeff Rapaport MD FAAD Realself Hall of Fame Castle Connolly Top Doctor

is it true not all scars are treatable? I have a 10+ years old scar on back of my hand (Photo)

Asked By:AnonymousANSWERS (1)
Treating a 10-year-old scar with a combination of 5-Fluorouracil (5-FU) or corticosteroids, subcision, Platelet-Rich Plasma (PRP) therapy, and CO2 laser offers a multifaceted approach to scar revision. Here are the benefits of each treatment and their combined effects: 5-Fluorouracil (5-FU) or Corticosteroids - **Reduction of Scar Tissue**: Both 5-FU and corticosteroids can help reduce the size and thickness of hypertrophic and keloid scars by inhibiting fibroblast proliferation and collagen production. - **Anti-inflammatory Effects**: Corticosteroids can significantly reduce inflammation, redness, and swelling associated with scars. - **Softening of Scar Tissue**: These injections can soften the scar, making it more pliable and less noticeable. Subcision - **Breaks Down Fibrous Bands**: Subcision involves inserting a needle under the skin to break up the fibrous bands of scar tissue, which helps to release tethered scars from underlying structures. Platelet-Rich Plasma (PRP) Therapy - **Enhanced Healing and Regeneration**: PRP is rich in growth factors that promote tissue repair and regeneration, accelerating the healing process. - **Increased Collagen Production**: PRP boosts collagen synthesis, improving skin texture and elasticity, and reducing the appearance of the scar. - **Reduction of Inflammation**: PRP can help to reduce inflammation, promoting a smoother healing process. CO2 Laser - **Precision Resurfacing**: The fractionated CO2 laser targets specific areas of the scar, leaving surrounding tissue untouched, which minimizes damage and promotes faster healing. - **Collagen Stimulation**: It stimulates collagen production, improving skin texture and elasticity over time. - **Scar Resurfacing**: The laser helps to smooth out the scar by resurfacing the skin and reducing scar thickness and visibility. - **Comprehensive Scar Revision**: Each treatment modality addresses different aspects of scarring, from breaking down existing scar tissue to stimulating new, healthy tissue growth, providing a comprehensive approach to scar revision of your hand. Excellent - definitely will be coming back for further procedures

Came to see Dr. Rapaport for my acne scarring. I came in to the office with no hope thinking my face won’t look normal ever again. After having a consult with Dr. Rapaport and getting subscision/ PRP and infini done I can say I feel more confident to take pictures and I can wear less make up. Dr. Rapaport gave me back my confidence. Not only did I have great experience with the procedure, let’s not forget how amazing and kind his staff is, all are very attentive to your needs, I was very nervous but they’re very friendly and made me feel like I was in good hands. I’m very grateful for my decision in trusting Dr.Rapaport and his staff to help me with my Acne scar problem. Definitely will be coming back for further procedures and recommending him for his great service!!!!

Cheeks had great improvement - want more improvement on my forehead

I have always been extremely insecure on all my scarring and have spent countless amounts of money on procedures. After seeing minor results & doing ALOT of research I came across Dr Rapaport & decided to give it a go. Dr Rapaport and his staff were nothing short of amazing! After my first consult I scheduled my appointment for Subscision, PRP, and Infini. (For a busy Dr, they got me into his schedule within two weeks of my consult which was major points for me). Besides the numbing, everything was painless! I followed up with Dr Rapaport two days after in his Old Bridge office (again accomodated me as Old Bridge office was WAY closer to where i live!) My down time was about a week because you of course have needle marks where he enters the site but for me they healed rather quickly. You also have to suction the sites (he does give you a device) so the scar tethers don’t reattach. My first round definitely helped improve my cheeks most. After 4 weeks I went back to see him again for a follow up where he decided I should get another round done. By the time my second session came around and we analyzed my face and marked it up- We definitely didn’t have to do as many spots on my cheeks as the first round which definitely made me alone think the money spent was worth it. Second procedure was as quick and painless as the first and followed up with him same as my first procedure. Since my second procedure i was told to microneedle daily with a .3 roller and follow up with him in a couple weeks. I am now a little over six weeks post my second procedure and just visited Dr Rap again today for a follow up. We both agreed my cheeks had great improvement. My forehead definitely improved (will post pictures) But Of course my biggest concern is my forehead and I want more improvement (as we all do, haha!) he did recommend that now that we subscised we can go to filler. I am scheduling my appt for that to see him in the next couple weeks and will go from there. Overall, I definitely recommend Dr Rap. He has a ton of experience and has been nothing but very nice and knowledgable and has answered any question I’ve had. I really can’t thank him enough for giving me improvement in my insecurities that I’ve had for quite some time. I know I’ll never get perfection so any % of improvement is a win and money well spent for me. Even when I wasn’t in the office, I emailed Debra from his staff some concerns/questions and she even shared my email with him and they got back to me that same day. Nidia his assistant as well, Amazing! She was so sweet and caring and made me feel extremely comfortable as well. Thank You Dr Rapaport for Everything! Looking forward to continuing with you for all my procedures and needs! I added some photos but mostly of forehead as thats been my main concern for forever. Cheeks were less of my worries so I didn’t really capture any photos to check the improvement! Updated on 1 Jun 2022: Update- top is prior to all procedures done and bottom is five months post of 2 subscisions with PRP, RF Microneedling, and then Filler. Unsure if worth all of the money. Do feel like I am basically back to square one with minimum improvement.

I Have Great Improvement on my Face

I have acne scars for over 10 years. I did one PRP & Subcision in Dr. Rapaport's office, I can see my skin improved 70%. After a month, I did one treatment of Mirconeedling RF to reinforce the result. It helps more. My skin is glowing now. Scars gone, but there are some red spots on my face still, Dr said it will go away gradually, I cant wait to see the final result... It is good that Dr did follow-up appointments after each procedure done, which is helpful and important to me, I have many questions and concerns after every treatment and he removed my concerns.

Acne Journey. Subcision has truly released the tethered scarring

My acne/scarring journey is a long one, so I’ll try to condense it as much as possible. I am a 46 year old female. I started getting deep, cystic acne nodules in my late teens. Sometimes they would come to the surface of my skin so I’d pick endlessly, while other times they remained dormant, albeit sore and feeling like hard marbles. I tried every acne cream and product over the counter, all to no avail. As I entered my twenties I decided to take serious action, so I asked my local dermatologist if she would consider putting me on accutane (up until this point she had me try Retin-a, as well as tetracycline, however nothing was effective), and she agreed. While the 5 month course of accutane was effective at controlling my acne, I was left with rolling acne scars. In my thirties I was financially in a position to do something about the scarring, so I started with micro needling and subcision. While I feel like the multiple micro needling sessions helped with the shallower scars, beyond a shadow of a doubt it was the subcision as that truly released the tethered scarring that forms rolling, saucer type scarring. The key is to be patient and persistent! I have had 7 (or maybe 8?) subcisions during the course of 10 years with Dr. Rapaport in New Jersey. I can hear the nokor needle as well as the cannula, breaking up the fibrous tissue that bounds the scar in a “popping” sound. Dr. Rapaport accompanies PRP after he performs subcisions to promote further healing. After the 3rd day of the procedure I suction every day for 2 weeks. This helps to prevent the re-tethering of the scars, and after every subcision I get gradual improvement. PLEASE don’t give up or become discouraged with your scarring as something can be done about it! I’ve spent many years reading this forum as well as acne.org to see the latest treatments available, but the one thing I can tell you is that subcision is the best procedure if you have rolling scars. It has been a long, expensive journey, but I am grateful to Dr. Rapaport for his expertise and knowledge in the field of acne scarring.

Amazing Results Already!

I recently moved back from Chicago and was looking for a new doctor in the NJ area to help continue treating my acne scars. I had found Dr.Rapaport online from all his positive reviews in treating acne scars. Although, he is a hour away from me I wanted to give it a try and see what could be done for my acne scars with his treatment. I so far have had one single treatment done with him and it was subscision with PRP and was so surprised in how amazing my results were from one treatment with him. My scars have improved more than the multiple treatments I have had done with other doctors during my time in Chicago. I highly recommend him and his office.

I Wish I Came Here Sooner!

Excellent experience with consultation and first treatment. I have been wanting to see Dr Rapaport for years after reading reviews for his success in acne scar treatments. It's a four hour drive so I put it off and have seen local doctors with little results and some money wasted. Three days ago I had subscision with PRP and so far the results are amazing but it's still very early to see the end result. What I can say for sure is that he is incredibly knowledgeable and is a great doctor. His staff is equally as great. Everyone I dealt with was very nice and professional. I am so glad I made the trip to this office and I look forward to more treatments here. I will post pictures when the swelling and prp has settled!

Wonderful Doctor, Wonderful Team!

I would like to take this oppurtunity to express my graditude to Dr. Rapaport and his staff. I had been to multiple doctors to treat my acne scars and with one single treatment at Dr. Rapaport's office my scars have improved more than the multiple treatments I have had with other doctors. Dr. Rapaport is an expert in his field with acne scars and keeps up on all the latest break-through science for treatments. I highly recomend him and his office.

Acne scar treatment - I knew that I was going to one of the best doctors

I have had a consultation with Dr. Rapaport and he recommended that I get a combination treatment consisting of Nokor+Cannula subcision followed by Infini RF followed by Fractionated CO2 laser and PRP. Dr. Rapaport said that with my scarring (Pretty severe and across 75% of my face)(mostly rolling and boxcar), I would probably need two treatments spaced about 6 months apart.(to allow us to see full results of the first treatment before determining if a second one is needed.) The procedure wasn't bad at all. They give you laughing gas to relax you during numbing injections and during CO2 laser, but I opted not to use the gas as it wasn't really that bad. I had the procedure done 5 weeks ago and I keep seeing results day by day. As of 5 weeks out I would estimate my overall improvement to be somewhere around 60% on my left cheek, 70% on my forehead, 40% on my right cheek and 40% on temples. This is crazy to think about as during my consultation with Dr. Rapaport, I have told him that I would be happy with 30% improvement after the procedure. I remember he said, " If you will be happy with 30%, then you will be really happy!", and at first I didn't really think more than 30% in one treatment was possible. The reason I have chosen Dr. Rapaport is because I felt that he has tremendous knowledge in acne scar revision. You can tell even if you know nothing about acne scar revision or have never done your research. The amount of confidence he has in what he does only comes with years and years of experience. His staff is very good! Nidia was very nice and genuinely caring and his assistant Debbie was very nice and knowledgeable working alongside Dr. Rapaport. Dr. Rapaport himself is an amazing doctor who I felt truly cares about the well being of his patients and does everything he can to get the best results for his patients without recommending costly and useless treatments that might produce mediocre results. The wait in the office was fairly long every time, but it did not matter to me, as I knew that I was going to one of the best doctors for acne scar revision.

Amazing Results and Experience

All throughout pregnancy and during breastfeeding my hair was as usual. After I stopped breastfeeding my hair became really thin and sparse. I was upset after all that I've gone through - I wanted my hair back. I started Scalp PRP injections at Dr. Rapaport's office in NJ. After 4 treatments done once each month my hair is thicker and there are no sparse areas. I am so relieved to have my hair back. It's so important for moms to feel like the women they are after pregnancy and breastfeeding. Thanks Dr. Rapaport and his great staff!

PRP & Dr. Rapaport Changed My Life!

Dr. Rapaport has changed my self-confidence, and my life. I started losing my hair when I was about 13 years old and I spent over 10k visiting different doctors, trying different "FDA approved" treatments (both topical and oral) and nothing worked. It wasn't until I heard about PRP that I started doing more research. I watched Dr. Rapaport's PRP testimonials, learned I was the perfect candidate for PRP & decided to take the chance. Dr. Rapaport not only took the time to sit down with me and learn about me and what I have tried, he explained the process to me. He is by far the most knowledgable doctor I have met, & I cannot stop raving about him (he even tested PRP on himself). I started getting PRP about 2 years ago & the first few times I went I saw no improvements, & stopped going, BUT they called me and asked me why I stopped coming and they said come back in lets try this a different way. NOW, many people could say this was to regain your business but Dr. Rapaport's office is always packed & one client does not effect his bottomline, this call was simply because "I am the perfect candidate & there is no reason why PRP wasn't working" I took the chance and went back in for another appointment, Dr. Rapaport & i sat down, we discussed my medical history, what medications I was taking, etc. & he created a custom PRP treatment for me, discovered what medications I should & shouldn't be on. What speaks volumes is that with an office full of people he is dedicated to each & everyone of his clients & their progress (he is not just in it for the money). The custom PRP treatment plan he created for me worked in just 2 treatments & my life has changed completely all thanks to DR. Rapaport! If you are struggling with hair loss, stop wasting money on treatments that don't work. If you are losing your hair, please do yourself a favor & go see Dr. Rapaport, he has literally changed my life, my attitude, my confidence & most importantly my hair is so no longer falling out, or thinning, it has grown significantly, what was once bald spots are now spots full of hair! (I used to live in NJ, now I live in FL & I fly to NJ to see him, trust me, it is worth the trip)!
