Concern of facial paralysis or smile alteration from Botox in masseter

Hi and thanks for your question. 26 units is a relatively small dose, especially for someone who usually gets 60 units. Unless you noticed that the person was injecting you in a very different place than usual, I think it should be OK. Actually, injecting into 4 places is more effective than injecting a large bolus of Botox into one spot. Where people run into trouble is if they inject too far towards the middle of the face. If your smile is affected, you would see signs of it within the first week after injection. Judging from the amount she injected, I would not be too worried if I were you.

It is unlikely that your botox treatment for masseter reduction will alter your smile negatively or result in facial paralysis. Techniques can vary, with some doctors using one injection over multiple. Placement is key to getting good results. Botox results take 1-2 weeks. For Botox injections the placement of the injections as well as the amount of Botox determine the overall outcome of the treatment.  On average, I would say that I would normally use 10-20 units of Botox in each side of the Massenter injecting in 2-3 sites.  Every injector has there own style and preference.  So far, it sounds reasonable so I would wait 1-2 weeks to see what happens and not worry too much about it.

However, your dosage is small and it should not affect it. On the other side, strategic injection with four small injections is more favored by me than one deep injection. It is very unlikely that Botox injected into your masseter would cause facial paralysis or alter your smile.  As long as it is injected into the main part of your masseters (which is the prominence you can feel near the angle of your jaw when you clench) it should be alright.  The masseter is not one of the muscles that affects smiling or facial expression.

There are 2 methods used to treat the masseter with Botox--one with 3 sites and one with 5. the one with 3 avoids the area closest to the lips and avoids the lip issue.  The dose of 13 units is probably too small to do anything and I suggest you go back to your dermatologist

Botox (like Xeomin and Dysport) is a medication injected into target muscles of the face to selectively relax them for fewer/softer wrinkles. It can also be used to decrease the size of the masseter muscle for conservative jaw slimming. The technique for injecting the masseter is different that the muscles that cause wrinkles. Some wrinkles are injected right under the skin. The masseter is a deep muscle that sits right on top of the jaw bone. There are nerves, muscles and glands sitting on top of it, so your surgeon needs to use a longer needle and meticulous technique to inject the masseter muscle appropriately. Just like any other Botox injection the onset starts around 3 days. It may extend to 7-14 days to see the relaxation of the target muscle. The shrinking effect on the masseter may take 6 weeks or more to see. The effect wears off when the medication wears off. I would recommend seeing a specialty-trained physician for this type of injection. Safety comes first.
