Which would you recommend; Chin Implant Or Sliding genioplasty and Rhinoplasty? (photos)

Question

My chin is weak and recessed and I can't make the decision for chin implant or sliding genioplasty. it seems like chin implant is more easy but sliding genioplasty can do more than implant. also Sliding genioplasty looks more nature. which one I should choose?i also PS one picture to show what result i want.I also want to make a rhinoplasty in the same time. is that possible? last question, I feel my lips is not wide enough and little bit asymmetry. is there any small surgery can fix it?

It certainly is possible to combine chin augmentation surgery with rhinoplasty. Like you mentioned sliding genioplasty can achieve more powerful results in that vertical chin height changes can be made in addition to bringing the chin forward.In my practice sliding genioplasty is done as an outpatient procedure, even if combined with rhinoplasty. It's hard to know if either chin implantation or sliding genioplasty could achieve the level of augmentation that you morphed in your photo. For the chin, you have a common problem that Asian chins tend to have, which is a tall chin but recessed. It appears that you have enough bone mass to get a reasonable advancement, although perhaps not to the degree of the photoshopped picture. A chin implant would not correct the excess height.For the nose, I agree that it's primarily a tip projection issue and will require strengthening of the cartilage of this area. If you have not had prior harvest, I prefer ear cartilage and sometimes septum to reinforce the tip cartilages.

I believe the best thing to do is to take X-rays of jaw and see if you need jaw surgery or just chin. If need jaw surgery, neither one of the treatments would be ideal.

The photograph demonstrates  a recessive chin which can be augmented with a chin implant for cosmetic purposes only, versus performing a sliding genioplasty for both cosmetic and functional bite issues. A sliding genioplasty is performed as an inpatient procedure at a hospital with 1-2 days stay overnight versus a chin implant which can be performed under local anesthesia as an outpatient procedure. Sean Frank, I use chin implants which are made in many sizes and shapes. You are right they are much easier and without the complications of a sliding genioplasty as along as the surgeon you choose is especially experienced in contouring faces with implants. Your type of request for the shaping of your nose is also not particularly complicated like many noses are. My own practice is to have patients like yourself send pictures of men where you like the shape of the nose and the chin and also do a computer imaging process in a consultation to be certain that your goals are communicated completely with the surgeon. Noses have almost no pain following surgery and chin surgery is uncomfortable only for a few hours. You will look like you did not have surgery after one to two weeks. You are an excellent candidate. Should your bite
require improvement due to orthognathic issues then you may want to consider
orthognathic surgery. Should this not be an issue I much prefer a silastic chin
implant to a sliding genioplasty. Less risk, better aesthetics (in my
opinion) and excellent long term results. Make sure you find a surgeon who
routinely performs this procedure, see lots of photos and hopefully get more
than one opinion. Good luck!

Yes, you do have a weak chin which would benefit from a chin implant.  If you drop your finger from  your lips straight down, your chin should just about touch your finger.  Your's is far behind.  Adding a chin implant will project the chin forward and better balance the lower half of your face.  Yes, you are correct, the chin implant is much easier.  In addition, it does work very well to add projection.

Dear Sean Frank, Genioplasty may be performed for augmentation of the chin or a chin implant may be used. During an examination it would be determined what the best choice would be for the patient depending upon whether the patient is having problems with bite issues. If there are no bite problems and it is just for aesthetic purposes then a chin implant is a relatively simple procedure to fix a recessed chin. Your imaging gives a fairly realistic photograph of proposed surgical results. During consultation your surgeon can perform digital imaging to show you the proposed surgical results and then discuss with you the surgical plan to achieve those results. Best regards, Michael V.
