With undetectable viral load and normal CD4 counts, your risks for surgery are the same as for any other patient. There is some risk of the surgeon and staff being stuck with a needle during the procedure and this does make one reluctant to take the risk. Generally we do assume that all patients are infective and we protect ourselves and take great care to avoid injury during surgery. But this may be the reason that you have found some reluctance. I think you should be able to find a well-trained board-certified plastic surgeon who is willing to do the surgery. More important is to check with your medical doctors that you are in good condition to have the surgery done and that your immune system is functioning adequately.

I have performed BBL surgery on many patients who are HIV positive with very good results. As long as the virus is undetectable and you are cleared by your medical doctor and you have enough donor fat you should be able to undergo a BBL.
