Fat transfer to buttock very unhappy with results - 8 months post-op. (Photos)

Question

I had a fat transfer to my buttocks in April of 2017. I am very unhappy as the fat seems to be placed just at the top of my butt, making it appear like a shelf and the bottom remains flat. Is the fat usually transferred everywhere or is the fat supposed to be localized at the top of the buttock? Can correction surgery be done? I also feel there was not an adequate amount of fat removed from my abdomen and back through liposuction. I have included one picture of my front and back pre and post.

This can be improved with a revision. Keep in mind, harvesting is important to fat transfer. I use water-assisted fat harvesting which gives pure fat to transfer and combine with platelet rich plasma (PRP) to get the most viable and sustainable fat cells. This can be combined with Sculptra to improve outcomes. I recommend getting a formal evaluation with an expert. Best, Dr. Emer

You might want to consider a revision to adjust where the fat is and perhaps add more in other areas to give you a better shape if you have the donor site.

BBL is not just about transferring unwanted fats from multiple sites to your buttocks.  An equally pressing goal is to achieve a more feminine figure, which is classically and even scientifically defined as having a waist to hip ratio of 0.7, meaning the waist circumference accounts for 70 percent of the hip width.

The good thing about BBL is that it can reshape the buttocks almost without limitations in terms of the injection location.  It can be injected into the lateral side (hips), lower buttock, and upper buttock.  This is not the case with butt implants that are invariably positioned above the sitting area (the upper half of the butt cheek) so the patients will not sit on their implants as constant pressure that may affect their long-term stability.

My BBL typically involves large volume liposuction of the flanks, lower back, and anterior abdomen to define the waistline.  However, over aggressive approach may not suit patients with poor skin elasticity due to increased risk of surface irregularities.  Also, a remarkably tiny waist is to be avoided if the goal is to achieve natural and proportionate results.

By eight months you may consider revision.  Just make sure that you consult a board-certified plastic surgeon who performs BBL on a regular basis.
