Hi! I am about to have asian rhinoplasty with local anesthetic only. I worry I’m going to freak out while on the table. I Will be taking Percocet an hour before the procedure. Is it common for people to freak out during local anesthesia? Thank you!
Answer: Rhinoplasty under local Great question! It really all depends on what exactly is being done. Asians tend to have a low and wide bridge as well as a bulbous tip. If you're only getting an implant placed to enhance the bridge then local maybe fine. However, if its more involved such as breaking the bones to narrow the bridge or if there is significant tip work that needs to be done, or if cartilage is being taken from another part of your body, then you need to be under general anesthesia. With that said, every nose that I have done have all been under general anesthesia. Especially if you are going to freak out, it is important to be asleep. Answer: Asian Rhinoplasty Under General Anesthesia Some cosmetic surgeries can be done under local anesthesia, however, I would recommend general anesthesia for rhinoplasties. I find that the nose is very sensitive and difficult to get the entire nose completely numb even with local nerve blocks and direct injection of local anesthetic. With that being said, there are some nasal standalone procedures that I will perform under local anesthesia such as Weir or sill excisions. If you are truly planning on having a full rhinoplasty involving your nasal bridge, and significant nasal tip work, I think you will be much more comfortable having this procedure done under general anesthesia. If you have had a recent Covid test with a swab in your nose, you probably experienced just how uncomfortable it can be even to have a Q-tip stuck into your nose.

Most patients now opt for Autologous techniques (e.g.DCF) and even if implants are used, the tip is reconstructed using septal +/- ear cartilage etc so general anesthesia would make more sense. Limited surgery of the Asian nose including dorsal augmentation and tip work can easily be performed under either local anesthesia or IV sedation plus local anesthesia. If septal, turbinate or extensive bony remodelling is in the cards, then either IV sedation or General anesthesia may be more appropriate. Discuss these options with your chosen rhinoplasty specialist, and once you are comfortable with the available anesthetic options, chose your best method of anesthesia that might allow you to achieve your new fabulous nose!

I would recommend at least using IV sedation, commonly called twilight sleep or conscious sedation. I've performed many this way and my patients do very well.
