A rhinoplasty procedure is not performed in the office. It is a surgical procedure performed in a sterile Environment in an outpatient surgery Center. There are lots of nerve endings in the tip of the nose and it is very difficult to get the entire tip numb to perform rhinoplasty under local anesthesia. In our practice, we perform a simple alar plasty under local anesthesia in our surgery center, however when any tip work needs to be performed, the patient should be placed under general anesthesia for patient safety and comfort in a sterile environment

Since these two structures do not involve alteration of any bony structures, you can perform these surgeries under local anesthesia.
They can be done under straight local anesthesia. I had that done myself when I was in my 20's.. That part of the nose is very sensitive, they did not buffer the anesthetic with bicarbonate, nor did they inject slowly nor do a nerve block first. It was very painful. With better injection technique and a strong narcotic pain pill 30 minutes before, it could be tolerable.
