Question

I’m a Caucasian female, late 30’s, 5’4, 150 pounds looking to have a mommy makeover. I want a tummy tuck with lipo, breast lift with implant exchange to a smaller size. I had two breast augmentations done in 2012 due to a capsular contracture in the right breast, resulting in a larger pocket and “double bubble.” Overall they still look good, but my left breast is starting to fall lower than my right breast. I had 750cc silicone implants placed. My goal now is to have smaller lifted breasts and downsize to around 400cc implants. I’m currently a 36DD and would like to be a small D or high C. I’ve had one virtual consultation so far and was told my breasts would be more of a revision and would need to be done separately from my tummy tuck and lipo due to the amount of time it would take to fix them. Is it possible to still have nice, smaller looking breasts? I’m worried I will end up with “botched” breasts. Is this a complex surgery that cannot be done with the rest of my mommy makeover? Your goals for a mommy makeover are very common, but given your history of capsular contracture, "double bubble" deformity, and the significant volume reduction you're requesting, your case does fall into the category of a complex breast revision. Downsizing substantially while performing a mastopexy requires meticulous capsulorrhaphy internally tightening the over-stretched pockets to ensure the new, smaller implants don't shift or bottom out. While many surgeons prefer to stage these procedures to prioritize safety and precision, it is often possible for an experienced, board-certified plastic surgeon to perform both the breast revision and the abdominoplasty in a single session, provided your overall health is excellent and the operative time is managed strictly to minimize anesthesia risks. The key to avoiding a "botched" result is ensuring that the internal pocket is structurally reinforced to support the new volume, so I recommend a second in-person consultation to specifically discuss internal bra techniques or the use of Acellular Dermal Matrix to provide the long-term stability you need for a beautiful, proportionate outcome.

I think it is definitely reasonable to do these two operations at the same time. Especially because, while I do see that you have some descent of your breast tissue on the left side, your nipples are in great position, so you may not need a full lift with the lollipop scars- perhaps just around the areola and along the base of the breasts, which would be better hidden. Thank you for your question.  I have been in practice for over 30 years and revisionary breast surgery is a large portion of my practice.  I have performed 100's of mommy makeovers similar to yours.  There is no reason that you cannot do them together.  I do have my own operative facility and have patienst stay overnight and then have hyperbaric treatment the next day.  This is all very routine for my practice.  You should be able to have an excellent result.  Best wishes to you.

It is best to see patients in person to do a full exam before offering a surgical plan, as the ultimate plan would depend on a variety factors such as anatomy, existing implant location, need for pocket switch/pec repair etc. The ability to do combination surgery (abdominal surgery + breast surgery) depends on the extent of each operation. Based on the goals you mentioned for your breasts and what can be seen of your abdomen, I think there are a number of surgeons who would tell you this could be done as a single operation. I would recommend finding a surgeon who's work you are happy with a go for an in-person or virtual consultation to discuss exact surgical plan for this.

Based on your nice post photos, I agree you can have all these surgeries done at the same time of full tummy tuck with muscle repair, liposuction, exchange of implants with possible breast lifting. My fear is 400 cc from 750 cc would take you to a C cup not a D cup. I would recommend at least a 500 cc implant and possible skin only lift that’s all can be done in one city expect fees to rage from $15,000-$25,000. Best of virtual consult with for better explanations.
