I have had moderate plus 533/575 cc impalnts for 15 years UTM. I am due for a new set and will be switching to OTM. I’ve dealt with years of muscle tightness and discomfort, and do not want to go through that again. I have also had pretty bad animation deformity. Should I Expect a very noticeable increase just by going OTM if I were to keep the same size? My plan as of now is to increase to moderate plus 650/695 because I don’t fill out my current size. I am 5’10” with a BWD of 16 and 160 lbs. Bra size with the current implants is about a 40B. But will that in combo with going OTM make too drastic of a difference? Do implants OTM look noticeably larger than UTM, if sticking to the same cc’s?

My practice has exploded with switching from subpectoral to subfascial implant pockets solely because of the advent of Motiva implants, the only device in the world that down regulates inflammation and the potential for subsequent capsular contracture. I am one of the 22 physician investigators for the Motiva USA trials that lead to their FDA approval and can tell you that the data is real despite curmudgeon surgeons saying its all PR. There is a small boost in apparent breast size with the pocket switch, but not to the degree you have planned (+/- 100 cc boost). For these difficult situations of upsizing like yours, I recommend making 100 cc rice sizers to see how you like the look with bra and clothes on. I would not factor the pocket change into implant selection. As a side note, your pectoralis muscle should be fully repaired by repositioning on the chest wall using your old capsule to secure the repair; be sure your surgeon has experience doing this.
