Can lift the tip of my nose with 2nd rhinoplasty?

Question

Hello, I got a rhinoplasty in December 2017. I still want to improve my nose. And specially the tip. I find it to bulbous and not lifted and defined. My “dreamed” nose would be with a refined lifted tip. I tried to include picture of this type of nose. Do you think I could achieve the look I am looking for? In order to evaluate most accurately- all views of the nose are needed. I would discuss with your Surgeon if further improvements can be achieved

Usually swelling can take up to at least a year and final results to be seen, and so time still may be on your side... one of the challenges in rhinoplasty is not to have the tip lifted too much which may occur...

Hi, I am sorry you are unhappy with your initial surgery.  Thank you for including the images of what you are trying to achieve. Some of your "goal" photos, especially the one you called pixie looking, are pretty unnatural looking and so I would caution you that things can be overdone and look too sharp at the tip, which will be worse than you started.  I think that you had a bigger nose that is now smaller and you have moderately thick skin.  You currently have a little bit of a "pollybeak" deformity where there is fullness above your tip.  I would suggest you discuss your goals with your surgeon.  I think that a moderate revision with tip sutures and possibly a tip graft could project your tip more, making your pollybeak go away and giving you the more rotated (upward rotated) and refined tip you are looking for.

WIth limited photos it is difficult to comment, but I would say if the tip is not at the angle you desire, then often raising the tip can be performed.  This can be treated with trimming or your caudal  septum and the tip cartilage as well. Best of luck.
