Please, please advise me, I had implant replacements due to capsular contracture. My surgeon told me following the procedure that the implant and scare tissue was worse than he had thought. I'm 16 days post op and the results are devastating, my breasts did not sag like this prior to the operation I'm concerned he has used an implant which is too small to replace the old one and scare tissue removed? I'm questioning why he would not have used a sizer. Please advise what my options are?

You are early in the recovery process. Use of a sizer would not have added much information due to swelling and other issues that happen with capsulectomy. Although online consultants will not be able to provide you meaningful reassurance regarding the final outcome of the procedure performed, I can tell you that the appearance of the breasts will change over the course of the next several months. Generally, patients who undergo this type of revisionary surgery should be informed that it will take several months for the skin and breast tissue overlying the breast implants to "take shape"; these tissues tend to have a memory based on the prior encapsulation. Although easier said than done, I would suggest that you continue to exercise patience. If in the long-term, you remain concerned about breast size/shape, additional surgery including re augmentation with larger implants and/or breast lifting may be necessary, depending on your physical examination and goals. Best wishes for an outcome that you will be pleased with longer term.

You are only 2 weeks into the recovery and healing process. Swelling after surgery, especially a capsulectomy may take another month to resolve. Continue to follow your plastic surgeons advice and continue with a good supportive bra. You will see your breast shape improving over the next several weeks and months.Good luck.

Answer: Disappointed after Capsulectomy It is too soon to be so disappointed. When there is a capsular contracture around an implant it pulls the implant upward (displacement). Once the capsule is removed the implant will sit in the correct position within the breast which is lower. To you, even the same size implants will look smaller. I always warn patients about that and if they are getting new implants I recommend at least 75-100 cc more. I learned this lesson early in my practice and the few patients that were unhappy were so because they wanted larger implants. You can't appreciate what your breasts are going to look like afterward so you have to appreciate that he/she didn't have much to go on. If you are unhappy in a few months I recommend you have an honest conversation with him/her about your disappointment. Chances are that you can just have your implants exchanged for larger ones, but you will be in a much better position to determine the desired size now that you can see where your implants are. Be patient while your tissues recover from your recent surgery. Like the first time things usually change for several months.
