Please, please advise me, I had implant replacements due to capsular contracture. My surgeon told me following the procedure that the implant and scare tissue was worse than he had thought. I'm 16 days post op and the results are devastating, my breasts did not sag like this prior to the operation I'm concerned he has used an implant which is too small to replace the old one and scare tissue removed? I'm questioning why he would not have used a sizer. Please advise what my options are?

Answer: Disappointed after Capsulectomy It is too soon to be so disappointed. When there is a capsular contracture around an implant it pulls the implant upward (displacement). Once the capsule is removed the implant will sit in the correct position within the breast which is lower. To you, even the same size implants will look smaller. I always warn patients about that and if they are getting new implants I recommend at least 75-100 cc more. I learned this lesson early in my practice and the few patients that were unhappy were so because they wanted larger implants. You can't appreciate what your breasts are going to look like afterward so you have to appreciate that he/she didn't have much to go on. If you are unhappy in a few months I recommend you have an honest conversation with him/her about your disappointment. Chances are that you can just have your implants exchanged for larger ones, but you will be in a much better position to determine the desired size now that you can see where your implants are. Be patient while your tissues recover from your recent surgery. Like the first time things usually change for several months.
