I have developed a rather complex placement of sutures in the neck that suspend the tissues up toward the lower face, thereby creating a very clean, sharp jawline. The texture of your skin should also be addressed with laser resurfacing either at the same time as the neck lift, or during another visit. Since your photo shows loose skin and loose playtsma muscle bands (ridges), with intervening depressions from the suction, I think a standard necklift to tighten loose skin, tighten upward the saggy muscle bands, and smooth out any irregular fat deposits under direct vision would offer you the best options to create a smoother, firmer neck contour. This would require an incision behind both ears and a short horizontal incision just beneath the chin, and could be performed with local anesthesia and iv sedation.
