What lift and breast implant shape and size would you recommend to get the look i want - dense, side boob, cleavage? (Photos)

Hi, thank you for sharing your question. You are a good candidate for breast augmentation and mastopexy. The exact sizing and type of lift incision is difficult to say by viewing pictures but you should be able to achieve your goal look.

It is certainly possible to get a more full upper pole by utilizing breast implants. The type, style, and size of implant would need to be chosen based on what is called tissue based planning. This technique requires choosing the correct implant based on your chest wall "base width" amongst other measurements. Based on these measurements, I would choose a moderate plus or high profile implant. A high profile implant will give you more upper pole fullness, while the moderate plus implant will give you more of a side profile.
In terms of the lift, I feel as if your lower pole will need significant reshaping. You will likely need a small horizontal scar along with the vertical scar, which would be more of a Wise pattern or "keyhole" lift.
In terms of the look you are going for, I think it is always important to remember that the goal is to improve the look of your breasts by correcting the specific anatomic problems, not to give you someone else's breasts. I think the upper pole fullness is achievable, but correcting the lateral breast curve is notoriously hard in breast rejuvenation.

Based on your photos, you need a procedure that will address the horizontal and vertical excess breast skin and tissue. This can only be achieved with an "anchor" or "inverted-T" final scar.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

Thank you for your question. You definitely would need implants AND a lift to get an attractive result. Often times, I will perform a technique I developed called Reductive Augmentation which also involves removal of extra beast tissue so I can use the implant to shape the breast. Based on your photos and description, I suggest you consider one of 2 approaches, depending on discussion of just what degree of lift you want. First, the simpler option is just to place a very large moderate profile implant in the subglandular plane behind your breast glands. This will enlarge and lift your breasts, just not as much as a true breast lift. The more extensive approach is what I call a Plus/Minus Mastopexy (breast lift). This includes placement of a moderately large implant behind the pectoral muscle, removal of some excess breast tissue in the middle just below the nipple, which is the tissue that tends to create more sag in the future, and then skin tailoring for the lift. The plus refers to adding volume via an implant which is the only effective long-term way to create upper pole cleavage fullness, and the minus refers to removing lower central breast weight that tends to sag earlier in the future. Your should be able to have a vertical or lollipop skin tailoring for the lift, but in my experience I can create a more rounded look and higher lift with a short inframammary fold incision added.

Augmentation would be a definite to approximate the photos you provided. You will likely need a higher profile implant, and if you want density, you should opt for a cohesive (gummy bear) implant.
Your nipples look like they are at the level of your fold. It is possible you may be able to just go with augmentation with a fuller implant. If you do need a lift, you would probably qualify for the lollipop.

I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. You need a full breast lift, you have tissues in the side, and your breast hang a lot, so you need a full breast lift and implants probably 550-600 to get the DD or DDD

You seem to be excellent candidate for breast lift and augmentation with implants. Considering that in the process of lifting patient usually looses 1/2-1 cup size, in order to achieve desired size of 34DD/DDD implants in the range of 320-400 ml. would be my suggestion. Very likely that vertical (lollipop) scar is all what is needed. In person consultation with board certified plastic surgeon with good reputation in breast surgery, is recommended.
