I have had 415 and 450 cc implants for 3 years, no children, no weight loss. Was a small a cup prior. Will I need a lift with capsulectomy and explant?

Answer: Capsulectomy should be done with explant and possible lift Removal of breast implants, intact or ruptured, may have positive implications if patients are symptomatic. A constellation of symptoms associated with breast implant illness, and autoimmune disease are considered when treating patients who have breast implants and are concerned about foreign body reaction. In our office, we begin this journey with a consideration of symptoms and exclusion of obvious autoimmune problems. In many of our patients, en bloc removal of breast implants with capsulectomy is important and beneficial. Further clinical evidence about breast implant illness is evolving and will continue to guide treatment. I would suggest removal if there is a possibility of foreign body reaction. Answer: Removal Your nipples are in a good position, but your implants have sagged from the weight. If removed, you will have extra skin. An examination is needed to see if the lift could be done at the same time as the removal, or would it need to be done later. If your breast tissue is very thin around your nipples now, a lift could damage the blood flow to your nipples and cause them to die. You might consider tightening your lower pocket and going down in size that better fits your frame and tissue. Remember, you were not happy with your breasts before implants and may not be happy once removed.

Answer: Lift or no lift with explant Based on your photos, I don't think a lift will be beneficial to you, as you nipple appears to be in a good position and there is not much tissue to lift. I recommend removing the implants, giving the tissues time to retract on their own, and then making a decision about a lift in a year or so.
