Thank you for your question and photos.  Every patient has different and sometimes unique anatomy.  While your breasts are divergent (all breasts are) the nipples are on the center of the breast mound.  The nipple needs to stay on the center of the breast mound or the shape of the breast does not look right.  An implant alone will only make your breasts larger.  It will not change your nipple position.  

Thank you for your question and photo post. Yes, you can improve your cleavage with implants. I would avoid a narrow implant base because it will give the impression that the nipples are even further spaced out. With proper positioning of the implant, you can improve cleavage as well as improving fullness under the breast.
