Ive always been petite, and my right one is like a cup size smaller than the left. My nipples get hard and “normal” looking when cold/exercising and I’m happy with them then, otherwise not so much.

You have a very mild degree of tuberous breast (lower pole constriction) based on the following:
1.  Areolar herniation
2. Short distance from the nipple to the fold

Because this is very mild, I would recommend a straightforward IMF incision/subpectoral approach with placement of conservative implants (250-350 cc).  This should create a very nice result.

I would not recommend a periarolar approach (as suggested by another PS) because of the increased risk for capsular contracture.

Your areolas are slightly herniated which is consistent with tuberous breasts but they do not look tuberous. I would recommend a circumareola approach to re-position the nipple-areola complexes more medial and at the same time place small implants. The implants will have to be different size since one breast is larger than the other. At size 32 or 34, each 100 cc of implant corresponds to 1 cup size change. From this, you can compute the volume required to achieve your desired goal.

Based on these photos you have mild breast asymmetry and protruding, herniated areolas. Both of these conditions can be treated in a single stage surgery. In person consultation with a board certified plastic surgeon would help. Good luck.
