I am 40 and have smallish breasts. While I like the look of my nipples, my breast shape bothers me. Would they be considered tuberous? Would I benefit from augmentation?

Thank you for your question. Tuberous (or constricted) breast features exist on a spectrum &mdash; the hallmark signs are a narrow, constricted breast base, a high or tight inframammary fold, a wide gap between the breasts, and sometimes herniation of breast tissue into the areola. Many women have only mild versions of one or two of these features, and from your description &mdash; small breasts with a shape that bothers you but nipples you are happy with &mdash; you may fall into that milder category. A definitive answer requires a physical examination, since the tightness of the lower pole and the position of your fold matter as much as how the breast looks in photos.
The good news is that even when tuberous features are present, augmentation can be very effective. In milder cases, an implant combined with internal release (scoring) of the constricted tissue expands the lower pole and creates a natural, rounded shape. In more pronounced cases, we sometimes stage the correction or add periareolar adjustments. Since you like your nipple appearance, a dual-plane augmentation through an inframammary incision would likely preserve exactly what you like while addressing the volume and shape concerns.
I would encourage you to have an in-person consultation where your tissue quality, fold position, and base width can be measured. With that information, your surgeon can tell you precisely whether a standard augmentation will suffice or whether any tuberous correction techniques should be incorporated. Either way, you sound like a good candidate for a meaningful improvement.
