Once swelling subsides and the tide is low the underlying work surfaces, there are several technical imperfections there, like over aggressive plication of tip domes, imbalance between middle vault and osseous dorsum, over projection of tip, probable miscalculated spreader grafts at the middle vault, too large columellar strut, etc. On top of that, your skin is quite thin and allows enhanced visibility of all that.

With that said, I don't believe you have a very bad work done, probably had you had thicker skin you'd be in an acceptable aesthetic situation, but in cases with thin skin the surgeon has to be a master of perfectionism.

Never steroid injections in a nose. They are destructive, they corrode tissues without control creating additional deformities, plus sometimes typical yellowish skin blemishes are possible.

You can't fix technical imperfection with just a magical shot, that's an unrealistic unicorn, unfortunately you'll have to run through the griev stages until acceptance of your situtation, and only then decide, with cold feet and calm emotions.

Hello and thank you for your question. Based on your photographs, your cartilage appears pressed against the skin and I would not recommend steroids. The exact cause entirely depends on the specific techniques used by your surgeon. Hopefully you can see hundreds of long-term before and after pictures with standardized lighting & backgrounds on your surgeon’s website which can provide reassurance. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

It can be difficult to tell exactly what is going on just in looking at photos. You have very fair, thin skin which can more easily show imperfections following rhinoplasty. These often do not become apparent until soft tissue swelling starts to resolve. Without knowing what was done during your revision surgery (graft placement etc), it is difficult to definitively say what is causing this. I tell my patients that it can take up to 2 years to see final results from a revision rhinoplasty, so you are still early in the healing process. I would recommend following up with your surgery so that they can do a full assessment and help you further understand what may be causing the increasing asymmetry that you are noticing.
