Nipple necrosis, infection, strange leakage. Will it heal in time? (Photo)

Question

It is obvious that I have some necrosis. What path will this take as it heals of does not heal? Ive been told it will "slough" what does this mean and what us the process of it like? Also you can see on one of the bottom stitches I have some strange clear/yellow/bloody continues leakage. It does not look open, hard to even see where it comes from. It does not smell. What is this and can it infect my other stitches? Will this necrosis affect the surrounding skin making it necrotic as well?

Thanks for your question and the photos. I'm so sorry that you are having problems. Normally when there is slough it is like a blister and the top portion of the skin separates. It usually heals from the inside out and the process is 4-6 week period. Often we are reluctant to remove the scab as it is acting as a bandaid. I normally have patients place Bacitracin ointment or Aquafor on the area to keep it from getting dried out. If there is thought to be superficial infection alongside the area then we prescribe oral antibiotics. The biggest risk is if there is an implant present, then it can get infected. Keep in contact with your surgeon and make them aware of any changes. Best of Luck!

Sorry for your struggles, do have an implant as well?  My honest opinion is that this is a serious issue and you and your plastic surgeon need to discuss all the options and possibilities.  If you use any nicotine-STOP.  I wish you the best of luck.

Hi MissangelladyI am sorry for your situation.This is very difficult, and will take a long time to resolve.The pics look like you have a total nipple loss.  I am concerned about the drainage and the redness around the suture lines.  Based on an exam, it might be prudent to explore the operative site and remove necrotic tissue.  It is my experience that your presentation has more necrosis below that can't be seen.  If true, the sooner that the necrosis is cleaned out, the better.You will likely need a graft to reconstruct the nipple and areola.Work closely with your ps to discuss what is next, time table, options, etc.sek
