Although you can make some adjustments to the nasal tip through a closed approach an open approach gives so much more flexibility, and precision. The scar is very very rarely an issue. I would strongly favor an open approach.

In general, closed rhinoplasty allows for less refinement of the nasal tip than the open technique, although there are excellent surgeons who do use the closed technique for tip work. Suturing, graft placement, changes in rotation and projection, and other maneuvers are more precisely performed through an open technique in general. Closed revisions allow for alteration of the nasal bridge and less aggressive tip changes. Yes, a revision rhinoplasty can be performed with closed approach, as long as the surgeon is very familiar with that technique. It is possible to reduce the bulbous tip that was not addressed properly with open rhinoplasty through a closed approach by making all the incisions on the inside of the nose. Yes a revision rhinoplasty can be performed using the closed technique on a tip that was not refined enough the first time. I prefer the closed technique over the open technique because I don't like the scar which is on the columella and I have done all my rhinoplasties with the closed technique and have had excellent results. These are rhinoplasties since the 1970's up to now.
It is possible to revise the nasal tip using the closed technique but more often than not an open procedure is used to achieve the best possible result.
