Nostril scarring, cartilage extrusion after revision rhino infection. Would removing warped graft help? (Photo)

Question

Hi, I had an alar rim graft pseudomonas infection after surgery 19 months ago as a result I have damaged, scarred tissue in my nostril, skin damage, dent and lump externally. It still feels inflamed, skin feels very tight in nostril from scarring and abnormal and there is barely covered cartilage poking out. I have a visible graft down the same side of my nose. Would this be best left alone or is there a surgical solution. Whereas the other nostril feels normal and healthy, the mucosa feels unhealthy.

After reviewing your photos, I believe you have two options:1. Steroid injections to reduce the hypertrophic scarring.  This may improve the appearance, but not likely resolve it completely.2. Excise the excess scar tissue and reposition the alar rim graftI recommend relaying your concerns to your surgeon, or if you don't feel comfortable, consult with a board-certified plastic surgeon who will accurately examine your nose and formulate a plan tailored to your needs. Of course I would agree that the best advice is with your original surgeon.  For what is it worth, based on the photos that you have submitted, I would leave things alone.  Most of the issues you are addressing can only be seen by looking up into your nostril.  Externally, things are not that bad.  I would caution you that removing and repositioning cartilage in a nose that already has significant scar tissue carries significant risks that things could turn out for the worst.  With time I suspect this will continue to improve on its own.

KAPM, this is really something that cannot be adequetly addressed with photos over the Internet. You really need to see several facial plastic specialists with years of experience and let them see and more importantly feel the tissue. Bring along a copy of your original operative note if possible. Your original surgeon is also a good one to at least get some opinions from. Good luck!
