What is a safe & permanent fix for filling a very deep radix? My Revision Rhinoplasty surgeon is recommending Gore Tex sheets.

I prefer using cartilage grafts to fill in a low or deep radix. If your current graft is out of position it is often possible to reposition it back up in the radix and use sutures to help keep it in place until the graft heals in position. I also use fascia grafts to fill in the radix as well. Your best bet is use of cartilage grafts.  There are a number of advanced techniques of radix grafting including use of diced cartilage and tissue glue.  Goretex implants are OK in patients with thick skin but can be problematic in thin-skinned patients.  Implants can also get infected.  See the case below for example of radix grafting.

I'm sorry to hear about your experience.  I usually do not use implants or foreign material to build up the nose to avoid the complications of infection, migration, extrusion, rejection, etc.  I prefer to use cartilage from within yourself to build the bridge to give a more natural result.  This can be done simultaneously with endoscopic sinus surgery if you see a facial plastic surgeon.  Please consult with a board certified facial plastic surgeon who can assist you with achieving the results you seek.  3-dimensional computer imaging can also help you visualize what you may look like afterwards and serve as an important communication tool with your surgeon.

In our practice, we do not recommend Gore-Tex placed in the nose due to complications that are frequently encountered with that product in the nose, such as infection. The best option for building up the radix especially in revision rhinoplasty, is the patient's own cartilage. Endoscopic sinus surgery can be performed at the same time by an ENT/ facial plastic surgeon under one anesthetic with one recovery period.
