Tired, sad looking eyes - Bleph with fat transfer or infraorbital implant? Ptosis? (Photos)

Tired of my tired looking eyes. Would love to know if a bleph with fat transfer would work here or would an implant be more suitable. Also does it look like I have ptosis and would addressing it better this sad Sam look. need a general starting point to ask the right follow up questions with my doctor of choice. (Still searching)

From your photo, the “tired/sad” look usually comes from a combination of upper lid heaviness, under-eye hollowing, and sometimes a subtle lid position issue (ptosis). You’re smart to ask for a starting point, because these are different fixes for different causes.
1) Do you look like you might have ptosis?
Possibly. If one upper lid sits a bit lower and covers more of the iris, that can create a sleepy or sad look. Ptosis repair (if present) is the most direct way to “open” the eye — and it’s different from simply removing skin.
2) Bleph + Fat Transfer
This is often the first-line approach when:
There is excess upper lid skin and/or a heavy lid fold
There is under-eye hollowing (tear trough) contributing to tiredness
Fat transfer can restore softness, but it needs an experienced surgeon because the under-eye is delicate and sometimes requires micro-fat/nano-fat techniques.
3) Infraorbital Implant
An infraorbital implant is usually reserved for:
True bony deficiency of the under-eye/cheek support
Patients who need structural projection that fat/filler can’t provide
It’s not “better,” just more structural — and it’s a bigger commitment than fat or filler.
Best Questions to Ask in Consult
Do I have ptosis, and would I need ptosis repair with (or instead of) bleph?
Is my lower lid issue hollowing, bags, or laxity?
Would you recommend fat transfer vs filler vs implant, and why for my anatomy?
Can you show examples of patients treated for a similar “tired eye” look?
I’d consult with a qualified, board-certified oculoplastic surgeon or facial plastic surgeon experienced in eyelids and under-eye volume work. That exam will quickly tell you whether ptosis is a key driver and whether volume restoration is best done with fat, filler, or structural support.
