47, female, hypermobile traits. I'd like a lower bleph (trabsconjunctival) with minimal skin pinch to remove early malar bags How risky is this for someone with skin laxity likely a form of hypermobility

You would be a great candidate for lower lid blepharoplasty. This surgery is typically done from inside the eyelid (transconjunctival) and fat is either repositioned or grafted to fill in the hollows. Low risk for hypermobility patients.

You look to be a good candidate for lower lid blepharoplasty; most likely this can be performed from inside the eyelid (the transconjunctival approach).  As for the malar area, there does appear to be some early malar edema there.  Have you ever had hyaluronic acid fillers injected there (like Restlyane, Juvederm, Volbella, etc)?  If so, these can be dissolved by injecting an enzyme.  Otherwise, the could be lessened by additional treatment at the time of your blepharoplasty (such as laser resurfacing, radiofrequency, or cauterization).  I would recommend seeking a surgeon who is a member of the American Society of Ophthalmic Plastic and Reconstructive Surgery (ASOPRS).
