Can the appearance or cankles be minimized with liposuction? What can I do to achieve more feminine ankles and legs?

Liposuction can most definitely be performed to the lower extremities. Because the layer of fat is thinner in this area compared to other common areas of liposuction (abdomen, upper thighs) we usually use smaller cannulas in this area and perform the liposuction carefully to prevent contour irregularities.

As you describe, “cankles” refers to when the ankle and calf are bulky and lack definition to create the aesthetically pleasing transition/contour from calf to ankle. Many women are self-conscious about the appearance of their lower legs and also often find it hard to wear boots. There are many things that contribute to the appearance and shape of the lower leg, including your bone structure, muscle tone, and fat. When fat is the culprit, liposuction can be performed to remove this excess fat and create a natural and aesthetically pleasing contour of the lower leg. I perform this using a combination of liposuction techniques with fine cannulas through small incisions under IV sedation in my fully-accredited operating room.

Liposuction below the knee or liposuction of the calves and ankles is a relatively uncommon procedure.
Many plastic surgeons don’t offer it.
I’ve been a board-certified plastic surgeon for over 22 years. The last 10 years of practice have been devoted exclusively to doing only liposuction and fat transfer.
I’ve done over 8000 and liposuction procedures.
I have treated calves and ankles less than 10 times in my life.
The procedure is technically difficult and the recovery is far more difficult than on other parts of the body.
Gravity causes immense swelling causing patients to not see their results for a long long time.
Patient should be prepared to have to do leg elevations intermittently on a daily basis for many months after surgery. certain shoes and boots may not fit properly for months.
I told most patients who get Liposuction final results will be present in three months and that recovery takes 3 to 6 weeks.
For calves and ankles you can multiply this times three. it will take nine months before your results are final and recovery can take 9 to 18 weeks.
The area is technically difficult and getting results without any contour irregularities or indentations especially working around the Achilles tendon is challenging.
The procedure should only be attempted by those who have truly mastered Liposuction.
The majority of patients to see me requesting the treatment or turn down. I will do the procedure and patients who have truly undesirable fat distribution in which the procedure is warranted. simply having chubby ankles is not sufficient in most cases for me to recommend the procedure.

Personally despite my experience as being a true specialist and Liposuction I’m reluctant to do the procedure and do it only on patient who genuinely need to have this and will have a clear benefit.
Individuals who is fat distribution it’s only slightly undesirable it’s probably not worth the risk of having contour deformities and an unpleasant recovery for the modest improvements the procedure and give.
It is a difficult and challenging area to do Liposuction.
