Hello @saha232. thank you for your question. In the subject of rhinoplasty what we seek is to achieve a natural result regardless of ethnicity. A natural result is not that it's going to look the same as your nose before surgery. A natural result is that the proportion and harmony of the parts that make up the nose tip, dorsum and nasal base are in accordance with the characteristics of your facial features. For example, a person with a wide face who has a wide nose, we have to modify it in such a way that it continues to be proportioned to that wide face. Many times in the street, we see men and women with very thin noses and absolutely large and prominent cheekbones, so that when it does not look harmonious. Best wishes!

Hi @saha232. Thank you for sharing your photos. I recommend seeing an expert rhinoplasty specialist for an in person examination. There are options for you that can be determined after examination. In my practice, I utilize piezosurgery. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures.

Based on your photo, you are a great candidate for rhinoplasty. The bridge can be built up and the tip and nostrils can be made more narrow. Realistic expectations are important because your skin is a bit thick, but a nice improvement can be made by an experienced rhinoplasty surgeon.

A detailed conversation with your surgeon is important to determine exactly what your goals are, as well as an explanation of what would be involved in attaining them. For most Asian noses, typical maneuvers entail augmenting the dorsum, refining the tip, and narrowing the ala, however, all this hinges on what exactly you'd like changed. You would be under the category of Asian rhinoplasty. In your case you will likely need a Tip-plasty and an Alarplasty.
Tip-plasty and Alarplasty

You can have the lower half of the nose (tip and nostrils) addressed without breaking the nasal bones. Tip-plasty can address the bulbous tip either through closed or open approach to make it narrow and refined.
Alarplasty is a very fine and quick procedure that can narrow the nostrils and reduce the flare to soften the appearance. It has other names as well like ‘Weir Excisions’ or ‘Nostril Reduction’. However, it is important to note that asymmetry of nostrils is common and cannot be corrected with alarplasty.
Both of these can be done under local anethesia.

Ethnic categories for rhinoplasty are somewhat overlapping and not a specific indication for surgery. They are most valuable for general orientation rather than procedure planning on an individual basis.

Consideration of your profile appearance and the characteristics of the nasal skin are important parts of the decision making. In most instances, the nasal septum is insufficient for additional augmentation in patients of South-East Asian descent. Costal cartilage graft (rib cartilage graft) is often required to produce more defined appearance in combination with reduction in thickness of the nasal tip skin.

A consultation using computer imaging and simulation to understand your goals both from the front and side views would be necessary if you want to define the general parameters of the surgery.

Many Filipinos have “ethnic” noses with thick skin and sometimes weak and wide tip cartilages. If you’re looking to achieve nasal narrowing and refinement, Rhinoplasty may be considered. While I cannot make a complete assessment of your nose from only a frontal photograph, your nose does appear to have a flat and wide dorsum, weak tip support and a wide base. You would most likely be a very good candidate for an ethnic rhinoplasty. Building height to your nasal dorsum (bridge) and adding support to your nasal tip would be key components to your rhinoplasty. This often requires additional cartilage grafts (e.g., rib cartilage) to reconstruct and stabilize the nose. This is best done with Open rhinoplasty approach. It is important to seek opinion from a Rhinoplasty expert, especially one that has experience with ethnic rhinoplasty. The nose is a three-dimensional structure, therefore a full set of facial photographs from all angles are going to be required to make a determination about how best to proceed. Digital computer imaging would also be helpful. A closed rhinoplasty approach can accomplish refinement of the nasal tip, and narrowing the bridge line.
