I am 8 months post op and am wondering I have a strong lower caudual septum which is pushing my columella down. Can this be fixed using closed? I feel like the tongue and groove technique has to be an open rhinoplasty? I understand trimming the lower caudal septum may fix things but if it has already been trimmed may he have to resort to the tongue and groove? I think it needs to be raised 2 or 3mm.

It is tough to say from your description without seeing photos, but in general, it is absolutely possible to widen an over-narrowed tip. You may or may not need actual ear cartilage to replace what was done as many times the tip is narrowed with sutures that can be removed. If you do need...

If there is persistent nasal swelling in the bridge area just above the nasal tip (supratip), dilute steroid injections may be helpful. When needed, we typically start with Kenalog 2.5-10mg/cc, and reassess 3-4 weeks later. Wishing you well. Dr Joseph
