To achieve more fullness on top, there a few different options to consider. If you were to exchange the saline implants for silicone implants, I would recommend considering an implant with more cohesivity (e.g. Motiva Round implants, Natrelle INSPIRE Cohesive implants, Mentor BOOST implants). These more cohesive implant create more upper pole fullness because they are firmer and maintain their dome shape better, regardless of whether you are standing or lying down, than the less cohesive implants. Another factor to consider is the profile of the implant. You may or may not still have the information card about your current implant, but changing the implant to a lower profile can also potentially create more upper pole fullness. Lower profile implants have a wider diameter, so they fill out the width and top of the chest more but don't create as much projection outward. I can't tell from the photo you submitted, but based on a physical examination including views from the side and measurements, your surgeon can also provide recommendations for changes in volume, position of the implant, and whether or not a simultaneous lift would be recommended.
