Even when I'm not having an active allergy attack my eyes look swollen, have deep bags, fine lines/wrinkles and generally look like an 80 yr old's eyes. I'm open to any option to improve the look (fillers/resurfacing/surgery) but not sure what is available to me with allergies. Currently I take a pill, a nose spray and eye drops to control my allergies.

Answer: Allergy related skin laxity Thanks for your photos. Your skin condition is often noted with severe allergies. This is loss of elastin resulting from skin swelling and inflammatory surface trauma from rubbing. I agree with Dr. Portuese that the allergies must be in remission before surgical blepharoplasty, otherwise the results may be disappointing. Consider collaborating with board certified allergists and surgeons to achieve a smoother result that is more appropriate for your age and appearance. I would recommend stromal vascular fraction injections in addition to surgical blepharoplasty to enhance your skin's collagen and elastin.

The excess puffiness and skin can be removed to rejuvenate the eyelids only after the allergies are under control

Your eyes to make you look older than your age and would probably be improved by conservative upper and lower blepharoplasty. Fat transfer from the lower bags to your orbital rims will be of assistance as will removal of a pinch of skin followed in the future by light TCA peel to reduce your pigmentation. These procedures should not be performed until your allergies are stable since swelling is not treated or corrected by surgery but may only be made worse.
