Based on the picture you appear to be good candidate for upper and lower back, remove those hate handles and take the fat we get and transfer to the buttocks and hips, to mold them and get better volume, contour and butt projection, is that we know as "Brazilian Butt Lift" (BBL).

Liposuction of your abdomen, back and flanks should give you more curves and hourglass shape and fat transfer to buttocks should give you more projection. 500 cc should be enough, since you already have some volume. Hi, it depends on the results you're looking for and even though you describe them it is difficult to tell you what 500 cc's of fat will do. You are going to lose a certain percentage of fat. It may differ in different individuals but you will not keep 500 cc's of fat. Now your fat content is reduced to 3 or 4 hundred cc's. Small amounts of fat make smaller differences. Florida has put in place regulations on BBL's due to an excessive number of poor quality results and sometimes very dangerous or worse results.
Additionally, some people may compare having surgery out of their home state to that of buying a car in another state. It is best to choose a local dealer and repair shop right down the road with a person you have a relationship with since you are spending a great deal of money and they are happy to help you if you should have some small things that need to be looked at when you take ownership of the car. If you bought your car out of state, say Florida and you live in the Northeast, you would call your Florida car dealer and tell him the problem. He would say well I'd be happy to help you if you want to drive the car down to Florida otherwise you need to find someone where you live. I don't know if you would be happy having to find someone else after you trusted the person that sold you the car and now they're not there when you need them. Best of luck.

Fat transfer is a great and natural way to augment the shape, volume and contour of many sites including the buttocks. Fat is a living cell and after the transfer process it must go through a healing period where not 100% of the fat transferred will "take". This is normal and expected but can also be a source of need for a second round/"touch up" fat grafting. That said, when we perform BBL surgery it is "not only what you add, but also what you take away." With the proper technique, even just liposuction of the flanks and lower back will give some effect of a BBL because it cinches the waist, defines the upper border of the gluteus and creates a better shape. With that and adding 500cc to each side, in my opinion, this will be "enough" to create a very noticeable and natural improvement in almost all patients, as long as expectations aren't for "the biggest backside in the world". Dear Reliable646441, thanks for sharing your concerns will be good to see other view pictures like sides, however if you are looking to get a natural profile 500cc could be good, keep in mind that after BBL procedures some of the fat transfered could be lose during the recovery time. Regarding the BBL, are you able to share with us why only 1L of fat can be taken? Is this due to regulation or something else? Here in Maryland, the max extraction of fat by regulation is 5 liters, or 5000 cc for accredited surgery centers. In answer to your question, an injectable quantity of 500 cc will provide a difference. However, you should be specific of where you prefer the fat to be distributed first.
Another question to consider is that it is possible that less than 500cc would be available to be injected from the 1000 cc. This is not an exact science, and everybody's fat is different. Some patient's fat has a higher % of oils. Once this fat is decanted, the volume of usable fat could be lower.

Another point to bring up is the questions of recovery. Here in my practice in Baltimore, I have been performing multiple procedure makeovers for 30 years. However, the only cases I do not combine are BBL with a Breast Procedure. This is NOT due to technical difficultries during the procedure, but rather because of the RECOVERY. With the BBL you should be recovering on your stomach. This is somewhat contraindicated for the recovery of the breast. There are cushions that have holes for the breast that can help, but it is always my protocol to do these procedures seperately to get the best possible recovery. Just be sure to discuss the best way to recover with your surgeon before the surgery.

As significant part of what makes the BBL such a great procedure one done correctly on patients who are good candidates is the Liposuction aspect.
For most of my patients Liposuction probably represents 60 to 70% of the improvement and fat transfer is probably only 40%. Fat transfer in addition to Liposuction definitely improves the outcomes but Reducing the love handles sufficiently and transitioning the contour from the buttocks to the waist well by itself, can have massive potential positive impact on body contour.
Liposuction can make hip tips be removed by removing fat just above the hip dip area creating an hourglass figure.
From what I see in your picture you don’t really need volume what you need is an improved contour.
In that sense Liposuction is more important and the addition of 500 mg of fat is probably sufficient. It seems to me you need volume, in the lateral part of your buttocks. Individuals who have substantial volume to start with typically need shaping more than they need volume enhancement.
In the hands of the right provider, a BBL limited to 500 mg should turn out an exceptional outcome.
It’s always ideal to not have restrictions, and in my opinion, the Florida rules are draconian and are not in doctors or patient best interest.
Forcing patients to have two operations is unnecessary and I think these rulings are unfortunate.
