Hello, I have silicone implants 275cc under the muscle 2 months ago along with breast lift but I'm not happy with the result. This black spot is skin necrosis on my areola. My breasts look way smaller than expected and the cleavage is huge. Please have a look at my current photos and expectations and let me know if 375cc will take me there. Please also tell me if I need more lift.
Answer: Going Larger Implants for More Cleavage Thank you for the photos, as they help. At this point given that you are very early in the recovery phase and you appear to have some wound healing issues, I would say you should wait at least 6 months from your original surgery. The one thing to take into consideration is that obtaining cleavage has a lot to do with the anatomy of the patient in addition to the surgery and breast implants themselves. A larger/wider implant may get you closer than you are now, but it is unlikely going to get your breasts as close as the photos you have shown. Also the photos that you have listed are all straight augmentations, meaning no lift was done. Therefore, it is difficult to compare the photos you have up to what you should expect from your surgery. You do have a very nice result, although perhaps on the smaller size for your liking, I would recommend allowing things to heal and then consider looking for a revision.Good luck.

Answer: Revising Breast Implants with a Size Increase Increasing your implant size requires adjusting the #pocket. Also, breast #lift also may be necessary; to better #support the new implant. Additionally, a mesh for further support may be helpful depending on the #implant size and skin #elasticity. With both implant types there are choices when it comes to size, shape, and profile. The criteria best to suit you will be discussed in full detail at your breast augmentation consultation. Any cosmetic #revision has risks of unhappy outcome or other complications. Also, the right implant for the revision is contingent on shape of body and your desired outcome. Your procedure should always be performed by a #PlasticSurgeon who is board-certified and has a great deal of experience specializing in cosmetic #surgery. You will then greatly improve your chances of getting the result you desire, and, without the need for another revision surgery. It is suggested that you look at before and after photos of the surgeons actual patients, and read patient #reviews. Gathering all of this information will help you make a well-informed decision.

Answer: Areolar spreading after breast augmentation I am sorry you are not happy with your result from breast augmentation. There are a number of issues here. The first is size. Clearly the size is not what you expected. A larger implant would help but it must have a base width considerably larger than the one you have, if you want to have anything approaching a 'cleavage.' I am afraid that to get a natural cleavage you would need much larger implants: an extra 100cc just won't hack it. Cleavage is not only dependent on the implant size and shape: it is also related to the position of your breasts on your chest; the shape of your ribcage and whether the implant is placed in front of or behind the muscle. The former is better in this regard because a sub-pectoral pocket is always limited by muscular attachments. There are some drawbacks to a sub-glandular placement, however. The other issue is the size of your nipple-areolar complexes: they appear to have stretched out after the augmentation. They will be stretched even more with larger implants. A Benelli (donut) mastopexy (breast lift) would help here by reducing the diameter of the nipple areolar complexes, and the area of necrosis could be removed at the same time.

Answer: Wait you need to wait for things to settle down after this initial surgery. You are still healing at 2 months post-op, we all realize this is not the result you wanted, but trying to rush into place bigger implants likely isn't going to help you or your situation.

Answer: Breast implant revision It is perfectly fine to undergo a revision to a larger size, BUT, you must wait several months for your tissues to heal, stretch, and the implants to settle into their permanent position. At that point if you still wish to go bigger I would encourage you to undergo a periareolar mastopexy simultaneously which will greatly improve the appearance of your breasts and areola. Your cleavage can probably be improved somewhat but that ultimately depends on the size and width of the new implant and your muscle/sternal anatomy. I hope this information has been helpful to you!Thomas Taylor, MD, FACSPasadena, CA
