Having lost 90 pounds, I would predict that you, will likely need plication of the rectus muscles to get a flat stomach whether you have a divarication or not!It is in the rare patient, with perhaps washboard abs, no fat and severe stretch marks, that this would not be carried out. I would go as far as to say that I would not plicate such a patient even if she had a divarication.

Since your capsules were loose and had to be tightened, it is common sense that maneuvers to stretch them such as massage are contraindicated for now. However, if you were to develop some firmness indicating a capsular contracture, then I am sure your surgeon would want you to start massaging them again.

Yes, you have some necrosis of the nipple-areolar complex. In the case of a breast reduction, this is a well recognized complication and the recommended treatment is to manage it conservatively with dressings, as you are doing. You would expect it to heal with a slightly widened scar, which could be revised later. The situation is more complicated if an implant is involved. Here, the danger is of the surface bacterial contamination of the necrotic wound gaining access to the pocket containing the implant. Once this happens, the implant will need to be removed. For this to happen, however, the necrotic tissue would likely reach all the way down to the implant. If there is healthy tissue between the wound and the implant then the conservative management might work here too. I wish you luck.

Thanks for your interesting question!O.K. somebody has to say it: you are too thin for this modality! Consider a silicone implant. It is safe, easy, cheaper and less likely to cause problems. Seek out a Board Certified Plastic Surgeon.

The scar in arm lift surgery extends longitudinally from the apex of the armpit down the inside of the upper arm. It hardly ever crosses the elbow joint into the forearm or reaches the armpit itself. However, following massive weight loss – due to gastric bypass or extreme dieting – all bets are off. In such cases the scars frequently cross these boundaries simply to taper the results and avoid unsightly dog-ears.

No Plastic Surgeon likes to hear the word "botched" particularly from his or her own patients! Simply not liking the result of an operation does NOT mean it was botched (ie. performed in a clumsy or bungling manner). Sometimes there is a simple miscommunication between patient and surgeon about what is wanted. Sometimes unforeseen and unavoidable complications can arise that affect the final result; and occasionally forces come into play that are are unique to the individual patient and which impinge on the healing process. The vast majority of Plastic Surgeons want to see their patients happy and are only too willing to revise the result to their liking. Frequently such revisions can be performed under local anesthesia in the office. Personally, I tell all patients that revisions are part of Plastic Surgery and that I will perform them if necessary to get the best result possible. So, speak up! Tell your surgeon what specifically you don't like and ask him/her to fix it!

Thank you for your inquiry. Do not worry: a solution is at hand! You have large nipple-areolar complexes and slightly tubular breasts. A simple augmentation might improve the shape of your breasts; but if nothing else is done, the nipple-areolar complexes will stretch out and become even larger. My approach would be to carry out a Benelli breast lift at the same time as an augmentation. This technique removes a donut-shaped piece of skin from the outer areola. Closure with a permanent 'ring-block' suture reduces the areolar diameter and resists stretching as well as elevating the nipple height and correcting the mild tubularity. I am sure that this would lead you to be proud of your breasts and more comfortable in those intimate moments you describe.

Congratulations on your new baby! Please do not consider breast surgery until at least 3-6 months after you stop breast feeding! Having said that you certainly do have grade 2-3 ptosis (droopiness) and this is unlikely to resolve spontaneously in my opinion. The good news is: you do not have tubular breasts. When things settle down you may consider an augmentation, a breast lift or both. The size of your nipple-areolar complex can be adjusted at that time if it is still a problem. And, yes, do consult a Board-Certified Plastic Surgeon. 'Double' Board Certified Plastic Surgeons such as myself are rapidly becoming a thing of the past with the introduction of integrated training programs. The American Board of Plastic Surgeons lays down rigorous training requirements and ensures that all their diplomates meet the same high standard. Double, triple and quadruple board certifications sound impressive but mean nothing if one of them is not Plastic Surgery! Good luck with your new child and your recovery from pregnancy.
