Would I benefit from facelift/ neck and eyelid surgery?

I’m 55 years old, thinking about having a facelift and upper and lower eyelid surgery. I mostly hate my fat neck. My BMI is normal, but always had thick neck.

There are many patients who due to anatomy have never experienced the look of a defined jawline; add the effects of aging and some feel they don't connect with the person they see in the mirror. Being able to help create that connection for patients is my passion. Based on the photos provided I would recommend a deep plane face/neck lift, upper bleph, brow lift. My suggestion is to opt for a procedure that returns the support to your face. The best in facial rejuvenation surgery techniques is the deep plane, which does not help to improve the support of your face from the muscle layer, obtaining natural and lasting results. Thank you for submitting your question and photos. You are likely a candidate for an upper blepharoplasty, as well as a facelift that also addresses the neck area. An upper blepharoplasty can improve a hooded appearance and create a more awake and refreshed result by removing excess skin and modifying the fat in this area.
During a facelift procedure, excess fat can be removed from the neck and jowls, the underlying muscles can be tightened, and the underlying tissue repositioned. Tightening the neck muscles can give the area under the chin more definition. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon who can conduct a physical examination and determine which combination of treatments may be right for you.

What you are describing regarding the neck contour is unfortunately an element of genetic characteristics that we don't understand. It tends to respond best to the deep plane neck contouring.

Based on the comments, in my opinion you could consider either necklift, fat transfer and upper and lower eyelid surgery or facelift and necklift, upper and lower eyelid surgery.

The option of using facelift and necklift as opposed to necklift and fat transfer is associated with more significant reduction in the contour of the jawline. This can produce an effect of highlighting the eyes because of the reduction in surface of the lower face.

If this is the type of outcome that you would like to proceed, then the combination of facelift and necklift may be the better choice.

Regarding your neck, your fat deposits are located both above and below the platysma muscle in your neck. Liposuction is an integral component of a lower face and neck lift, however it can only remove the compartment of fat above the muscle. The majority of your fat is located underneath the muscle which is genetic, and this must be surgically removed. A platysma plasty is also performed to tighten the neck muscles after the fat has been removed. In addition to fat removal in the neck, a face and neck lift accomplishes tightening loose facial and neck skin tightening the SMAS and lifting the jowls. The goal of upper eyelid surgery is to remove the excess skin creating the hooded look, while lower eyelid surgery involves mostly removing fat bags creating the puffy look. Blepharoplasty surgery and a lower face neck left can be performed together under one anesthesia with one recovery period. Look for a facial plastic surgeon with extensive experience rejuvenating the neck. The simple answer is yes.
A necklift only addresses the neck skin laxity. It will not improve the jowls or cheeks. A mini-lift  and a midface-lift also vary significantly in what is done, depending on the surgeon performing it. They are usually performed in patients with minimal laxity and would not improve the neck. The terms facelift and lower facelift mean different things to different patients and vary in their extent from surgeon to surgeon. A facelift or a lower facelift which would include the neck area would have the potential to produce the best cosmetic result of the neck, central cheek and jowl area. The eyelids can be cosmetically improved with a consrvative blepharoplasty. Micro-fat grafts can produce a long term improvement of the areas that are volume deficient.

It is important to realize that following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do based on limited photos without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest. I would suggest you find a plastic surgeon certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. You should discuss your concerns with that surgeon in person.

Facial rejuvenation needs to be individualized. What would be an ideal approach for one patient is not necessarily the same for someone else. Based on the examination and discussion at the consultation, you should have a better understanding of the various options that can achieve the best cosmetic result for you.
