Question

Im replacing my recalled textured saline implant due to grade 4 capsular contractor. I have had two consults Tone doctor said no lift needed. The other said he prefer to do a lift lifting the nipple 2 to 3 cm. I doing the surgery because the pain and hardness in my left breast I really dont have the extra money for this let alone adding a lift. But I also do not want a horrible out come.. please help any adivise from qualified dictor would be appeeaciated Thank you in advance

Thank you for your questions and pictures.  I agree with Dr LoMonaco's answer.  As long as the implant you use to replace is similar in size or even slightly larger, you do not appear to need a lift.  You should end up with a very nice and natural result even with the removal of the contracted capsule.  Be sure to consult with a Board-Certified PS for your surgery.

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

The best candidate for a breast lift has a low nipple, a long breast skin fold, and hollowness at the top of the breast they wish to have filled in.  Your photos indicate that you have none of the above.

A simple implant exchange with whatever size you desire, plus the scar tissue removal would probably suffice and get you a nice result.

Based on the submitted photos you should have an acceptable result with implant replacement of about the same volume with no lift.  The only exception would be if you desire for the nipple position to be higher.  That would require a lift.
