Chin implant vs sliding genioplasty - which procedure would be right for me? (Photo)

I am looking to get rhinoplasty and some form of chin augmentation. I have read about the pros and cons of both, but I am not sure which would be right for me. I am in my late 20s and still play contact sports so the use of my own bone rather than an implant appeals to me, but more providers in my area offer implants. I also don't want to further deepen the crease between my lip and chin. Which procedure would be right for me?

We almost always recommend a genioplasty over chin implants. They are more predictable, and the bone will actually heal stronger after the procedure (good for sports). By moving your chin slightly forward and down, your profile will have more balance and and added contour. A genioplasty will also add length and definition to your jawline (while also tightening the skin), which an implant will not. The labiomental fold (the crease below the lip) will move slightly, but it will not become deeper. Your side profile photograph demonstrates an overly projecting nose with a dorsal hump, and an under projecting chin. In our practice, we perform many chin implants in combination with a closed rhinoplasty. If you're only wanting to perform a chin implant, that can be performed under local anesthesia which takes about 30 minutes. The implant is  inserted through a small incision underneath the Chin. A sliding genioplasty is performed by an oral surgeon in a hospital setting under general anesthesia it with an overnight stay at the hospital and is much more invasive. It's usually performed when the teeth are significantly out of alignment. Digital computer imaging of both your nose and your chin would be helpful to understand what can be accomplished with both procedures upon your facial features. Both procedures can more or less offer the same outcome. My experience has been the plastic surgeons tends in the slightly more towards the use of implants while the oral surgeons come to be more prone sliding genioplasty. Oral surgeons 10 to see things more for my facial skeleton perspective. While plastic surgeons tend to be a little more soft tissue oriented.
You are correct in that forwarded augmentation of your chin with deep in your leave your mental fold or sulcus. 
That leaves only vertical augmentation with at most very small amount of anterior augmentation.

Your perspective is valid. It is important to note that your lower jaw appears 'recessive/underdeveloped' which is contributing to the 'deeper' labiomental fold concern. Without xrays, I would suggest a genioplasty performed under sedation/outpatient office will improve the 3D features of the chin area/including the jawline area. One also must consider the 'vertical' dimension and a genioplasty is superior in that regard as well. At your age, an implant is not ideal
