I have done masseter Botox but it hasn't gotten me to my goal face shape (the last three pics) I cant tell if it's fat causing my lower third to be round and heavier or if a jaw thing. I was thinking either sliding genioplasty or liposuction but I can't figure out what would get me to my Goal?
Answer: Genioplasty I think you have a few options. You appear to be a good candidate for a genioplasty. It looks like you have done some research on your own so you have a general idea of what that entails. It would help to define your jawline, create more definition and bring balance to your profile. You could potentially add jawline contouring or liposculpting to create even more definition. Another thought would be buccal fat removal. This will help to reduce some of the fullness in your cheeks (people will sometimes transfer that fat to other areas, like under eyes or cheeks). In all, I would say a genioplasty with jawline lipo and buccal fat removal will help to get you close to the aesthetic goal you are looking for.

Answer: What determines facial identity? Are the primary determinant a facial aesthetics structure. It is your facial skeleton to determine your appearance your facial make up. Soft tissue coverage cans to be a fairly consistent from person to person. What determines if someone is attractive or not. It is all based on bone structure. Soft tissue solutions are going to be inherently limited because they do not treat the underlying primary cause. Having an under sized mandible does not automatically get fixed by having chin augmentation. Liposuction, it’s not indicated because you don’t have an abnormal fat distribution. If you want a comprehensive facial assessment, then consider consulting with both oral surgeons and plastic surgeons. Oral surgeons sent to see things more from a facial skeleton perspective while plastic surgeons center see things more from a soft tissue perspective. There is overlap between the two specialties and both specialties contribute to facial aesthetics. Be careful with permanent irreversible treatment, such as buccal, fat pad removal, which tends to cause issues down the road when precious facial volume becomes a thing that is highly desirable. There is something to be said about accepting your own heritage and genetic make up, which was a gift from your parents.
