Risk at second gynecomastia surgery?

Hi Ohadunive, Thank you for your question regarding a second gynecomastia surgery. Based on your photo and not a thorough physical examination, I would suggest breast skin and fat removal surgery. Since you have excess breast skin you need breast skin excision. For your revision gynecomastia surgery, I would not suggest local anesthesia.  There is too much scar tissue and it will be too painful for you to tolerate under local anesthesia.  General anesthesia is better. Please seek a board-certified plastic surgeon with significant experience in gynecomastia surgery.  Travel if you need to. Thank you for sharing your question.  At this point the decision to be made is yours.  After liposuction the only remaining option for additional correction is direct glandular excision.  Without photographs or an in-person examination assessing the amount of gynecomastia relative to remaining adipose tissue it is difficult to render an opinion on the possibility of contour irregularity.  Discuss this with your surgeon.  Best wishes. 

Without photos, I can only give you general feedback. It sounds as if liposuction alone didn't deliver your desired results. So, yes, the next step would be to surgical remove the excess glandular tissue. This needs to be done by someone with experience and training as you don't want to take too much tissue nor too little. So, make certain that you see a board certified plastic surgeon. Best of luck.

Hi ohadunive,Thanks for the post but without a photo I can only give you general information.  Most patients who need revision because they feel their chest is still too full will need excision of tissue as opposed to just liposuction. This needs to be done in a careful way to not under-resect, but more importantly not over resect. This is why it is important to go to a board certified plastic surgeon who specializes in gynecomastia surgery. In my practice approximately 25% of the patients are have had the surgery before and they each need a very individualized surgical approach. It is not a cookie cutter operation!Sincerely,Dr. Dadvand
