Is it not possible for a surgeon to make the nose smaller and feminine? I am disappointed that my nose still looks big (Photo)

Sometimes thicker skin can cause the nose to not narrow as much as anticipated. That being said, you can still achieve additional narrowing and reduction to the tip region as well as the bridge of your nose. You may have to have some cartilage grafting from the inside of your nose to create more a tent pole effect to support and narrow the tip region. 

Hello, thank you for your question and provided photos. Based on them yes, you can have smaller and feminine nose you desired by a rhinoplasty procedure. You can benefit from alar base reduction to have smaller nose from frontal view, cartiage graft can be used.  You seem to have thick skin, to get the best result from rhinoplasty procedure, you need to treat yout skin by visiting a dermatologist.Regards,

Your first revision helped to some degree however, even in thick skin and likely a decent amount of scarring in the nasal tip I think that with properly placed cartilagenous grafts you can have a more defined and feminine tip and softer dorsum.  I agree that the bridge needs to be either closed or rasped to get rid of the prominent ridges and widened high dorsum.  A physical examination would help me determine which manuever is necessary.

In order to give you the best assessment - it would be helpful to see all pre-op and post op  photos (all views), including reading the op-reports. In person exam is essential. Based on limited photos- it appears that in the right hands- improvements can be achieved through a closed technique with a utilization of cartilage grafting. I suggest that you seek an experienced Board Certified PS with extensive Rhinoplasty and revision Rhinoplasty experience .

 Since this will be her third rhinoplasty procedure, it will be a significantly difficult operation due to the factors been extensive changes to the normal anatomy, thick skin and scar tissue present. An in-person examination is going to be required to make a determination about how best to proceed.  Make appointments with the 3 busiest rhinoplasty surgeons in your local area.

Dear Easygoing, I would speak with your revision rhinoplasty surgeon regarding your concerns. He/she knows best what techniques were used in your procedure and where you are in the recovery phase. You do have remarkable improvement from your original rhinoplasty however your nose is under rotated and needs slight shortening. You do appear to have an open roof deformity that needs addressing as well. If you do not get the answers you are looking for seek some secondary opinions with expert revision rhinoplasty surgeons. I agree that the pictures do show a wide bridge and poorly defined drooping tip. If you have allowed enough time since your last revision. I encourage you to consult with a very experienced surgeon who specializes in revisions because it will be difficult to get a smaller, more feminine nose with a third procedure.
