I have gone to 2 surgeons my primary one who did caudal septum trim 3 months ago2mm —(It didn’t shorten my length) my primary columella strut was done 1 year ago. I want to bring down the alar rims and shorten my nose. I went to 2nd surgeon 30 years experience and he blamed it on just having a short upper lip and didn’t wanna do what I wanted. I believe my caudal septum is longer then the average human vertical wise.I am arab. I want one more shot. Rhinoplasty can’t be done multiple times

After total healing and if the result is not pleasing either cosmetically or funtionally you cna go under a revision rhinoplasty.Regards,

Answer: Is it possible to shorten the vertical length of my nose and increase the space between upper lip and bottom of nose? Thank you for your question! Based on your pictures and question, I think what you are asking is reasonable. However, before making any other changes to your nose, you would need to wait at least 1 year to let all swelling resolve from your first procedures. In order to achieve the goals you would like to see in your nose, you will need a comprehensive revision rhinoplasty. What this means, is that you will need to have all areas of the nose addressed, rather than just the columella alone. A revision rhinoplasty specialist can and should perform a full examination, review your photographs with you, and then explain how this procedure would work. Based on your photographs, it appears you would benefit from having the tip rotated upward slightly, along with shortening the columella. In addition, placing alar rim grafts would help to bring the nostrils down, as well. Again, be sure to consult with a board certified facial plastic surgeon or plastic surgeon who specializes in rhinoplasty and revision rhinoplasty. Here you will have a full evaluation and be able to discuss your goals for surgery. I hope that this helps!-David Gilpin

Revision rhinoplasty as more difficult than a primary rhinoplasty due to the fact there has been alterations to the normal anatomy and scar tissue formation. A full rhinoplasty would be required to make the overall nose smaller and shorter

Answer: Shortening my Nose Based solely on these pictures i don't think you need to shorten the nose (increase tip rotation) but rather lift the columella and perhaps do alar rim grafts. Examination by a revision rhinoplasty specialist is necessary.
