Eye bags or malar fat pads? (Photo)

Question

What do I have under my eyes? It is very prominent since I was about 25. It depends on the lighting and if I'm swollen or not. I'm 36 now. Is there any surgery that can help me to remove that? No fillers , I'm interested in surgical procedures only.

From the photo, it does look like the orbital fat is pushing out some. This can be removed with lower lid surgery, and often the condition is genetic and can be seen in younger patients. At the same time, a fat transfer should be done to fill in the crease and make it look smooth. Be sure to see a facial specialist for consultation.

It sounds like you are asking about correction of lower eyelid/cheek groove correction? A transconjuctival lower blepharoplasty may be a good option. In person consultation with a board certified plastic surgeon familiar with this procedure is recommended. Good luck.

Dear Mandy,
You have tear troughs, the presence of which can be due to several things, descent of the malar fat bags, progression of orbital fat pads, combinations of both.  The correction (surgically) can range from fat transfer into the troughs to lower blepharoplasty with or without fat translocation or grafting.  Seek consultation with an American Board of Plastic Surgery board certified plastic surgeon for an examination and discussion of your options.  Good luck!

From the very limited photograph, it appears that you have fat bags in the lower lids creating the puffy look. A trans-conjunctival approach lower blepharoplasty can accomplish improvement with the incision located completely on the inside of the eyelid. No external insertion is required. This is performed under a brief general anesthesia as an outpatient procedure
