With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). At the same time, I would recommend lasers (Fraxel, CO2, Halo, or Erbium) and microneedling RF (Fractora, Morpheus8, PiXel8, Intensif) to improve the crepey skin and wrinkles. Overall, the neck can be treated with a combination approach:

Yes based on the pictures you appear to be a good candidate for chin/jaw liposuction to improve your jaw and chin angle also I recommend you the buccal fat removal. I would suggest to be treated by board certified plastic surgeon with experience in this procedure, to get the best desired results.

it is hard to tell for sure without an examination. Based on your photos, it looks like you would benefit from chin liposuciton. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

From the photographs presented, you appear to have fat deposits located both above AND below the platysma muscle in the neck. Liposuction can only accomplish removal of fat deposits above the muscle. Consider a surgical neck lift to remove the fat deposits below the platysma muscle in addition to liposuction above the platysma muscle and a platysma-plasty to significantly improve the jawline. No skin removal is required. Liposuction alone will only give you minimal benefit.

Fat distribution is not your problem neither is having an excessive amount of subcutaneous fat.

Your problem is based on bone structure and if you want to treat this correctly and treat the primary problem then you need skeletal manipulation.

Most likely the best procedure is a jaw advancement.

If you opt for a treatment option that doesn’t treat the primary problem then expect partial improvements at best.

This includes Liposuction, Kybella, chin augmentation etc.

When the mandible is small in comparison to the Maxilla we typically see the following.

Profile pictures both show an upper lip that has more forward projection than the lower lip. This is directly related to the alignment of the teeth which would be in a dental overbite occlusion. The lack of projection of the mandible causes the origin of the platysma muscle to start closer to the neck giving it an oblique angle. This intern gives a premature double chin, early lack of jawline definition and early jowls.

Some of those describe soft tissue conditions but the underlying cause a reason for them is still based on bone structure.

I think it’s worthwhile including a couple of oral surgeons on the list of doctors you should get consultations from.

It doesn’t mean you need to have cranial facial surgery but at least having a clear understanding of the problem and knowing all treatment options makes you a better informed consumer.

In the end the price for the procedure is what it is. Nobody has to have cosmetic surgery.

The most expensive cosmetic surgery are ones that don’t deliver improvements or worse.
